在肾脏疾病中检测血清Cystatin C的意义

摘　　要：目的 :探讨肾脏疾病中检测血清胱蛋白酶抑制剂以C(CystatinC)浓度评估肾小球滤过率 (GFR)的价值。方法 :应用酶联免疫吸附法 (ELISA) ,测定经临床及实验室检查证实有肾脏损害患者的血清CystatinC浓度 ,同时测定血肌酐 (Scr) ,尿素氮 (BUN)和血 β2 -微球蛋白 (β2 -GM) ,采用Cockcroft -Cault公式计算肌酐清除率 (Cockcroft) ,并用同位素99mTc -DTPA测定肾小球滤过率 ,血清CystatinC浓度分别与其它指标进行相关性分析。结果 :血清CystatinC浓度与β2 -GM (r=0 .69,P <0 .0 1) ;Scr(r=0 .63 5 ,P <0 .0 1) ;BUN(r=0 .5 49,P <0 .0 1呈正相关 ,与Cockcroft(r=- 0 .743 ,P <0 .0 1) ;99Tc -DTPA测定肾小球滤过率 (r =- 0 .791,P <0 .0 1呈负相关。结论 :临床上应用检测血清CystatinC浓度来评估肾功能是一种特异性高、准确性好、快速简单的方法。Objective:To study the evaluation of GFR in kidney diseases by detecting the Cystatin C in serum.Methods:Enzyme-linked immuno-sobent assay(ELISA) was applied to detect Cystatin C concentration in serum of patients with kidney damages confirmed either clinically or laboratorily. Using cockcroft- caula, the creatine clearance rate was calculated while the serum creafinine, blood urea nitrogen,and β 2- microglobulie (β 2-GM) had been detected. In addition, 99T- TDPA was used to assay GFR. The serum Cystafin C concentration was analyzed in correlation to other indexes respectively. Results:There was a positive correlationbetween serum Cystatin C concentration and Scr(r=0.635,P<0.01) ,Bun(r=0.549,P<0.01) , β 2-Gg(r =0.635,P<0.01); anda negative correlation with cockcroft(r=-0.743, P<0.01) , 99T-TDPA(r=-0.791, P<0.01).Conclusions:Detecting the serum Cystatin C concentration to evalute the renal function is dinically a fast, accurate,economic and effective means with a high specificity Moreover,the method is helpful in diagnosing the renal failure in an earlyperiod.
