维持性血液透析患者von Willebrand因子及可溶性E-选择素水平的变化

摘　　要：目的 :研究尿毒症维持性血液透析 (MHD)患者血浆vonWillebrand因子 (vWF) ,血清可溶性E -选择素(sE -selectin)水平的变化 ,并探讨其临床意义。方法 :应用双抗体夹心酶联免疫吸附法 (ELISA)测定 60例MHD患者和 2 0例健康对照组vWF ,sE -selectin水平 ,同时作颈动脉超声检测患者动脉粥样硬化情况。结果 :MHD组vWF ,sE -selectin水平显著高于对照组 (P <0 .0 1,P <0 .0 1) ,MHD中有颈动脉粥样硬化组vWF ,sE -selectin水平显著高于无颈动脉粥样硬化组 (P <0 .0 1,P <0 .0 5 )。MHD组vWF ,sE -selectin水平之间呈正相关 (r=0 .964 8,P <0 .0 1)。MHD组vWF含量与甘油三酯水平呈正相关 (r =0 .45 3 1,P <0 .0 5 )。结论 :MHD患者存在明显的血管内皮功能损害 ,vWF ,sEObjective:To investigate the changes of von Willebrand factor(vWF) and sE-selectin in uremic patients with maintenance hemodialysis(MHD).Methods: The levels of plasma vWF and serum sE-selectin were measured by enzyme-linked immunosorbant assay(ELISA)in 60 MHD patients and 20 healthy subjects. The carotid atherosclerosis of MHD patients was evaluated with B-mode carotid ultrasonography. Result:The levels of plasma vWF and serum sE-selectin in MHD patients were significantly higher than that in control group respectively (P<0.01, P<0.01, respectively), the levels of plasma vWF and serum sE-selectin in MHD patients with carotid atherosclerosis were significantly higher than that in MHD patients without carotid atherosclerosis respectively (P<0.01, P<0.05, respectively).Plasma levels of vWF in MHD patients were positively correlative with serum levels of sE-selectin (r=0.9648,P<0.01).Plasma levels of vWF in MHD patients were positively correlative with serum levels of hypertriglyceridemia(r=0.4531,P<0.05).Conclusion:The results suggest that there is obvious endothelial dysfunction in uremic patients with MHD, vWF and sE-selectin are valuable markers reflecting endothelial dysfunction in uremic patients with MHD.
