高血压患者社区综合防治管理模式近期效果评价

摘　　要：目的　通过对社区高血压患者综合防治效果的分析 ,评价模式的可行性 ,为探索适合于中国社区高血压病的有效防治途径积累经验。方法　在中关村社区组织“高血压之家” ,对这部分高血压病人采取综合防治措施 ,半年后总结干预效果。结果　经过干预 ,高血压患者收缩压和舒张压下降显著 (P <0 .0 5 ) ,整体血压水平收缩压下降了 4 .6 2mmHg ,舒张压下降幅度为 2 .11mmHg ,基线血压异常者收缩压下降了 10 .30mmHg ,舒张压下降幅度为 4 .77mmHg。高血压判定标准知晓率由干预前 6 6 .15 %上升到 74 .6 2 % ,显著高于干预前 (P <0 .0 1)。血压控制率从 5 9.6 2 %上升到 79.4 9% (P <0 .0 0 1)。对高血压患者绝对危险评估干预前后比较 ,干预后危险程度有不同程度的下降。结论　社区高血压患者的综合防治模式对有效降低血压。Objective To evaluate the effectiveness of community based comprehensive intervention for hypertension prevention and treatment, and to testify the feasibility of community based comprehensive intervention model. Methods Holding “family of hypertension patients” in Zhongguancun community in Beijing city. Community based comprehensive intervention for hypertension patients were taken up. After a half year, effects of community based intervention on hypertension were analysed. Results After a half year, the general means of both SBP and DBP decreased remarkably( P <0.05). SBP decreased remarkably by 4.62 mmHg and DBP decreased by 2 11 mmHg. After interfering by the model,these subjects whose blood pressure were above normal level had a remarkedly lower than before inerfering by 10.30 mmHg( P <0.05)for SBP and by 4.77 mmHg for DBP. The aware rate of the rule of hypertension was increased from 66.15% to 74.62%.Control rates of hypertension increased from 59.62% to 79.49%( P <0.001). It was showed that after intervention hypertension the absolute risk level was dropped compared with that before intervention. Conclusion The model of community based comprehensive intervention for hypertension prevention and treatment is available and feasible in community.
