高频超声评估盂肱关节下方关节囊厚度对冻结肩的诊断价值

摘　　要：目的探讨高频超声双侧对比评估盂肱关节下方关节囊厚度对冻结肩的诊断价值。方法高频超声测量25例冻结肩单侧发病者、30例肩袖撕裂单侧发病者及13例健康志愿者盂肱关节下方关节囊厚度(ICT),并对结果进行统计学分析。结果冻结肩组患侧平均ICT大于健侧、肩袖撕裂组及健康对照组平均ICT(P均<0.05);肩袖撕裂组平均ICT与健康对照组比较无明显差异(P>0.05);健康志愿者优势肩平均ICT与非优势肩比较无明显差异(P>0.05)。结论高频超声双侧对比评估盂肱关节下方关节囊厚度简单可行,对冻结肩的诊断具有重要价值。Objective To detect the value of inferior glenohumeral capsular thickening measured by high-frequency ultrasound in assessment of adhesive capsulitis of the shoulder.Methods Inferior capsule was evaluated by ultrasound in 25 patients with frozen shoulder diagnosed by clinician,30 patients with rotator cuff tears and 13 healthy volunteers,the differences were analysed statistically.Results The mean ICT of the affected shoulders in frozen shoulder group was greater than that of the unaffected shoulders,the affected shoulders in rotator cuff tears group and the healthy control group(all P<0.05).The difference of the mean ICT between rotator cuff tears group and healthy control group,and that between the predominant shoulders and the edominant shoulders in healthy control group were not statistically significant(both P>0.05).Conclusions It is feasible to use ultrasound to assess the inferior glenohumeral capsular thickening,and it is of great importance for diagnosis of frozen shoulder.
