摘 要:目的探讨声辐射力成像(ARFI)联合萤火虫成像(Micropure)在甲状腺恶性结节中的诊断价值。方法对62例甲状腺结节患者术行前二维超声、ARFI及Micropure检查,以术后病理结果为金标准,分别计算二维超声、ARFI、ARFI与Micropure技术联合对甲状腺结节良恶性诊断的灵敏度、特异度、阳性预测值(PPV)、阴性预测值(NPV)。结果二维超声诊断甲状腺恶性结节的灵敏度、特异度、阳性预测值、阴性预测值为73.68%、79.69%、51.58%、91.07%。ARFI诊断甲状腺恶性结节的灵敏度、特异度、阳性预测值、阴性预测值为84.21%、85.94%、64.00%、94.83%。ARFI与Micropure联合诊断甲状腺恶性结节的灵敏度、特异度、阳性预测值、阴性预测值别为94.74%、78.13%、54.55%、98.00%。结论 ARFI与Micropure技术联合应用对诊断甲状腺恶性结节有较高的临床应用价值。Objective To investigate the value of the acoustic radiation force imaging(ARFI) combined with Micropure technology used for diagnosis of benign and malignant thyroid nodules.Methods Sixty-two cases with thyroid nodules were examined with gray scale imaging,micropure technology and ARFI respectively before operation,taking pathological results as golden standard.Sensitivity and specificity,PPV,NPV of 2D ultrasound,ARFI,ARFI combined with Micropure method in diagnosing benign and malignant thyroid nodules were calculated respectively.Results The sensitivity,specificity,PPV,NPV for 2D ultrasound in diagnosing benign and malignant nodules were73.68%,79.69%,51.58%and 91.07%.ARFI were 84.21%,85.94%,64.00%,94.83%,and ARFI combined with Micropure were 94.74%,78.13%,54.55%,98.00%respectively.Conclusions ARFI combined with Micropure have highly clinical application value foe differentiation of benign and malignant thyroid nodules.
