32例胎儿肾上腺区包块的声像图分析

摘　　要：目的探讨胎儿肾上腺区包块的超声表现及诊断价值。方法回顾性分析在我院发现的胎儿肾上腺区包块的超声影像资料及出生后临床诊治结果。结果产前超声发现32例胎儿有肾上腺区包块34个,右侧18例、左侧12例、双侧2例。超声表现囊性20例、实性8例、囊实性回声6例,CDFI扫查内可见血流信号6例。出生后经手术治疗者9例,病理证实神经母细胞肿瘤3例、肾上腺出血2例、隔离肺2例、肾脓肿1例、肝囊肿1例;经保守治疗者23例,临床诊断肾上腺出血20例、隔离肺2例、重复肾1例。产前超声诊断包块为良性30例、包块恶性4例,其与临床诊断不符3例,符合率90.6%。超声表现囊性包块,病理诊断均为良性;超声表现囊实性或实性包块时,病理诊断可为肾上腺出血、隔离肺、肾脓肿或恶性神经母细胞肿瘤。结论产前超声检出肾上腺区包块以良性包块多见,产前超声检查可为临床诊治提供有意义的影像学资料。Objective To evaluate the ultrasonic manifestations and diagnostic value of fetal suprarenal mass.Methods We reviewed the files of patients with suprarenal mass retrospectively,the ultrasonic image data and the result of clinical treatment after birth were analyzed.Results Prenatal adrenal area masses were found in 34 cases,18 cases on the right side,12 cases on the left sides,2 cases were bilateral.The masses were cystic in 20 cases,solid in8,and mixed in 6.Surgery was performed in 9 cases after birth,histologic analysis showed 3 nephroblastoma,2 adrenal hemorrhages,2 pulmonary sequestrations.1 renal abscess,1 hepatic cyst.The clinical diagnosis were 20 adrenal hemorrhages,2 sequestration,1 duplex kidney in 23 eases with conservative treatment.Prenatal ultrasound diagnoses benign tumors in 30 cases,malignant in 4 cases,3 cases were unconformity with clinical diagnosis,coincidence rate was 90.6%.Ultrasonic manifestations of cystic mass,pathological diagnosis were all benign;Ultrasonic manifestations of cystio and solid,or solid mass,pathological diagnosis were adrenal hemorrhage,pulmonary sequestrations,renal abscess or nephrblastomas.Conclusions Prenatal ultrasound deteced suprarenal mass was more benign,prenatal ultrasonic examination can provide meaningful image information for clinical diagnosis and treatment.
