高强度聚焦超声治疗肌壁厚度不足3cm的子宫腺肌症的临床分析

摘　　要：目的观察高强度聚焦超声(HIFU)治疗肌壁厚度不足3 cm的子宫腺肌症的安全性及有效性。方法采用JC200型聚焦超声肿瘤治疗系统治疗30例子宫腺肌症患者,治疗后1、3、6个月对患者随访,比较治疗前后患者临床症状及超声影像学变化。结果 HIFU治疗后1、3、6个月分别随访30、26、24例患者,痛经、经量增多等腺肌症相关症状明显缓解,总体症状改善率分别为(36.00±17.67)%、(42.15±23.48)%、(37.33±22.69)%(P<O.05),腺肌病灶缩小率分别为(32.32±22.85)%、(38.50±29.26)%、(41.53±26.47)%(P<0.05)。所有患者均未出现严重并发症。结论 HIFU对于肌壁厚度不足3 c,m的子宫腺肌症具有较好的安全性及有效性。Objective To evaluate the effect and safety of high intensity focused ultrasound(HIFU) on adenomyosis with the wall of uterus less than 3 cm in thickness.Methods 30 patients definitely diagnosed as adenomyosis were treated with HIFU and the tumor therapeutic system of the HIFU model JC200.All women were given followingup at 1,3 and 6 months after treatment.The clinical symptoms,ultrasound images were compared before and after the HIFU treatment.Results 30,26,24 patients were respectively given following-up at 1,3 and 6 months after treatment.Dysmenorrhea and menorrhagia alleviate obviously.The total improvement of symptoms is(36.00±17.67)%,(42.15±23.48)%,(37.33±22.69)%(P<0.05).The average rates of reduction in adenomyosis volume were(32.32±22.85)%,(38.50 ± 29.26)%,(41.53 ± 26.47)%,respectively after HIFU ablation 1,3 and 6 months by evaluation of ultrasound(P<0.05).No outstanding side effect was observed.Conclusions The treatment of adenomyosis with the wall of uterus less than 3 cm in thickness by HIFU is effective and safe.
