实时三维超声与SPECT比较对急性心肌梗死介入治疗术后左心室功能评估的应用价值

摘　　要：目的比较实时三维超声心动图(RT3DE)和门控心肌灌注SPECT显像,对急性心肌梗死患者急诊介入治疗后左心室功能评估的应用价值。方法急性心肌梗死患者34例行急诊介入治疗后7d内同步行RT3DE与SPECT检查,并设立对照组,比较左心室功能相关参数LVEDV、LVESV、LVSV及LVEF。结果与对照组比较,RT3DE与SPECT测量的LVEDV、LVESV均明显增加(P<0.05),LVEF则明显减低(P<0.05);两种测量方法的参数LVEDV、LVESV均无统计学差异,SPECT测量的LVSV及LVEF均低于RT3DE测量(P<0.05)。RT3DE与SPECT的参数指标均有相关性,LVEDV及LVESV指标相关性最强,r值分别为0.772和0.768;LVSV、LVEF指标相关性较弱,r值分别为0.594和0.37。结论对急性心梗支架术后左心室功能的评价,RT3DE与SPECT测得的LVEDV、LVESV相关性好,具有相近的评估价值;而RT3DE与SPECT测得的LVSV、LVEF相关性较差。Objective To compare the RT-3DE and single photon emission computed tomography(SPECT)in assessment of left ventricular function in patients with acute myocardial infarction treated with emergency percutaneous coronary intervention(PCI).Methods 34 patients with acute myocardial infarction treated with PCI were enrolled in this study,and set up the normal control group.Real-time 3Dechocardiography(RT3DE)and SPECT was performed at the same time in seven days after operation.Left ventricular function parameters:left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular stroke volume(LVSV)and left ventricular ejection fraction(LVEF)were compared.Results Compared with the control group,the LVEDV and the LVESV of two groups were significantly increased(P<0.05),and LVEF was significantly lower(P<0.05).The LVEDV and the LVESV measured by RT-3DE and SPECT has no significant differences,but LVSV,LVEF has significant differences.Compared with RT-3DE group,The LVSV and the LVEF of SPECT were lower(P<0.05).There was excellent correlation between LEDV and LESV measured by RT-3DE and SPECT(r=0.772,0.768).The correlation between LVSV and LVEF was inferior(r=0.594,0.37).Conclusions Evaluated left ventricular function with treated with PCI,there was excellent correlation between LEDV and LESV measured by RT-3DE and SPECT.The correlation between LVSV and LVEF was inferior.
