三维超声造影对肝癌射频治疗的评价

摘　　要：目的探讨三维超声造影(3D-CEUS)对肝癌射频微创治疗效果的评价。方法对102例肝癌患者131个病灶进行超声引导下射频消融微创治疗,治疗前后分别进行3D-CEUS和二维超声造影(2D-CEUS)检查。结果治疗前3D-CEUS对肿瘤三维立体形态及肿瘤血管空间关系的显示率为92.3%,而2D-CEUS显示率为0.0%(P<0.05)。3D-CEUS对肿瘤滋养血管显示率100%,而2D-CEUS显示率为57.3%(P<0.05)。治疗后3D-CEUS对消融不全病灶残余血管的显示率为100%,而2D-CEUS显示率为9.3%(P<0.05)。结论 3D-CEUS可良好显示肿瘤立体空间形态、肿瘤血管空间灌注过程及肿瘤滋养血管,有助于临床精确制订消融方案,对消融术后疗效评估、随访有较高临床价值。Objective To investigate effect of 3D-CEUS in radiofrequency ablation(RFA)treatment with microinvasion for hepatic carcinoma.Methods The 131 tumors in 102 patients with hepatic carcinoma were treated with ultrasound-guided radiofrequency ablation.3D-CEUS and 2D-CEUS were examined before and after RFA treatment in131 tumors.Results Before RFA treatment 3D-CEUS to 3Dof tumors and space relation of blood vessel display ratio were 100%,however 2D-CEUS were 0.0%(P<0.05).3D-CEUS tumors display ratio of to nutrient blood vessel were 100%,however 2D-CEUS were 57.3%.After RFA treatment 3D-CEUS display ratio to remnant blood vessel of in completely ablation tumors were 100%,however 2D-CEUS were 9.3%(P<0.05).Conclusions 3D-CEUS can shown space form of tumors,space filling process of blood vessel and nutriment blood vessel of tumors.This will contribute to constitute ablation plan for evaluation of curative effect and follow-up patients.
