声触诊组织定量技术测量妊娠期高血压患者肾脏弹性的临床价值

摘　　要：目的探讨妊娠期高血压疾病患者的肾脏与正常妊娠孕妇的肾脏皮质、髓质及肾窦的剪切波速度(SWV)之间的差异。方法以90例妊娠期高血压疾病患者为研究对象,并取得部分于产后6周~3个月内做肾脏穿刺活检确定肾脏病理类型。依据妊娠期高血压疾病患者的分类标准,将妊娠期高血压患者为A组;轻度子痫前期患者为B组;重度子痫前期患者为C组;正常妊娠妇女为对照组。使用声触诊组织定量技术测量受检者的肾脏剪切波速度,检测其血压、尿蛋白和肾功能,并进行统计分析。结果 A组和对照组两者肾功能指标无显著差异(P>0.05);A、B、C组间的肾功能P值和B、C组间与对照组间的肾功能P值均<0.05。A、B、C组和对照组的皮质、髓质及肾窦的Vs值均有差异,P值均<0.05。结论妊娠期高血压疾病患者的肾脏的皮质、髓质及肾窦的SWV较正常妊娠孕妇的小,且肾脏损伤程度越严重,同一肾脏部位其SWV越小。Objective To investigate differences of the shear wave velocity(SWV)in renal cortex,medulla and renal sinus between pregnant women suffering from hypertensive disorder complicating pregnancy and healthy pregnant women.Methods 90 pregnant women suffering from hypertensive disorder complicating pregnancy were as subjects,parts of whom were given kidney biopsy to determine the type of renal pathology into 6 week to 3 month postpartum.According to standard of hypertensive disorder complicating pregnancy,patients with gestational hypertension were classified as group A;mild pre-eclampsia as group B;and severe pre-eclampsia as group C;Healthy pregnant women were in control group.To detect the blood pressure,urine and renal function of every subject receiving VTQ,then through statistical analysis the measured data.Results Compared with the control group,the renal function of group A showed no significant difference(P>0.05);There were significant difference among the indexes of the experiment groups compared those with the control group,the renal function in experiment group B and C had significant changes(P<0.05).The Vs of kidney cortex,medulla and renal sinus in group A,B,C were all significantly different from controls(P<0.05).Conclusions The value of kidney cortex,medulla and Vs of renal sinus of patients with hypertensive disorder complicating pregnancy were smaller than healthy pregnant women.And the more serious kidney injury,the smaller the SWV value in the same portion.
