高频超声对医源性周围神经损伤的诊断价值

摘　　要：目的探讨高频超声对医源性周围神经损伤的诊断价值。方法回顾分析32例医源性周围神经损伤的高频超声表现,分析其超声影像特征。结果 32例患者均有肢体接受手术等治疗的病史,且在治疗后均有神经受损的临床表现,其中桡神经损伤19例,腓总神经损伤5例,尺神经损伤4例,正中神经、股神经损伤各2例。高频超声可以清晰显示神经被金属内固定物等卡压、神经被缝线误扎造成的神经水肿、神经断裂及断端神经瘤形成。超声可以清晰显示受损神经与金属内固定物、手术缝线等周围组织的毗邻关系。结论高频超声可以清晰显示周围神经结构,可以判断医源性周围神经损伤的原因及神经损伤程度,为临床治疗提供重要信息。Objective To study the diagnostic value of high frequency ultrasound for iatrogenic peripheral nerve injury.Methods High frequency ultrasonic manifestation and characteristics of 32 cases of iatrogenic peripheral nerve injury were retrospective analyzed.Results All of the 32 patients had undergone surgical treatment on limb and complicated with the clinical manifestation of peripheral nerve injury after surgical operation.In the 32 cases,there were19 cases of radial nerve injury,5cases of common peroneal nerve injury,ulnar nerve injury in 4cases,median nerve injury and femoral nerve injury both in 2cases.The high frequency ultrasonography can show clearly that nerve entrapment caused by metal fixator,nerve edema caused by mistaken suture,nerve break and neuroma on the broken end.And high frequency ultrasound could exhibited clearly the adjacent relationship between injuried nerve and its adjoining metal fixator,suture etc.Conclusions High frequency ultrasound can exhibit the structure of peripheral nerve and judge the course and degree of iatrogenic peripheral nerve injury,which can provide important information for clinical treatment.
