摘  要:目的探讨急性心肌梗死患者冠状动脉病变程度,对左室心肌扭转角度及心功能的影响。方法使用二维超声斑点追踪技术分别测量左室短轴切面心尖水平和瓣环水平各段心尖旋转角度峰值、心底旋转角度峰值、扭转角度峰值等指标,分析冠状动脉病变程度对扭转角度及心功能的影响,并测量左室内径及左室容积等参数。结果 (1)与对照组比较,心肌梗死各组各节段扭转角度、左室整体扭转角度及LVEF均下降(P<0.05)。(2)直线相关分析,心肌梗死组冠脉病变程度与LVEF有较好的相关性,r=0.816(P<0.05)。(3)ROC曲线显示,以左室整体扭转角度15.0°为临界值,预测心肌梗死病变累及双支曲线下面积为0.846,预测左室射血分数<45%的曲线下面积为0.738。结论急性心肌梗死患者随冠状动脉病变程度加重,左室心肌扭转角度和心功能明显下降。Objective To study the effect of coronary artery lesion severity on left ventricular torsion angle and cardiac function in patients with acute myocardial infarction(AMI)by speckle-tracking imaging.Methods The peak rotation angle and torsion angle of each segment at apical level and annulus level in left ventricular short axis view were measured by 2Dspeckle tracking imaging.LV volume and LVEF were measured by 2Dechocardiography.The effect about coronary artery lesion severity on LV torsion angle and cardiac function were analyzed.Results Compared with control group,there were significant decrease in the each segment torsion angle and LV torsion angle as a whole(all P<0.05),while there were significant decrease in LVEF in AMI group(all P<0.05).Linear correlation analysis indicated there was a good positive correlation between the coronary artery lesion severity and LVEF after AMI,r=0.816(P<0.05).ROC curves indicated that when the critical value of the LV torsion angle was defined as 15.0°,the areas under the ROC curve was 0.846.When the critical value of the LV torsion angle was defined as 15.0°,the areas under the ROC curve was 0.738.Conclusions The severity of coronary artery lesions had serious effcet on LV torsion angle and the cardiac function after AMI.LV torsion angle may be useful parameter to predict the severity of coronary artery involvement and cardiac function in patients with AMI.
