摘 要:目的探讨应用应变率成像技术评价尿毒症患者左房功能的临床价值。方法 40例尿毒症患者,左室肥厚组(LVH)25例,非左室肥厚组(NLVH)15例及30例正常对照组。取心尖四腔、心尖两腔切面测量左房前壁、下壁、侧壁、房间隔心肌的收缩期、舒张早期、房缩期峰值应变率(SRS、SRE、SRA)及最大应变。结果 SRS:尿毒症患者LVH组的房间隔及左房下壁较正常对照组显著性减低;SRE:NLVH组仅有左房前壁较正常对照组显著性减低,LVH组左房各壁较正常对照组显著性减低;SRA:LVH组的左房下壁较正常对照组显著性减低(*P<0.05,**P<0.01)。尿毒症患者LVH组左房各壁的应变较正常对照组显著性减低(*P<0.05,**P<0.01)。NLVH组与正常对照组相比应变无显著性差异。结论应变率成像技术能够客观、准确地评价尿毒症患者的左房功能。Objective To evaluate left atrial function in patients with uremia by strain rate imaging and its clinical value.Methods 40 patients with uremia consisted of 25 patients with left ventricular hypertrophy(LVH),15 patients without left ventricular hypertrophy group(NLVH)and 30 cases of normal control subjects were enrolled in the study.Four cavity apex,two cavity apex were taken and for measuring left atrial anterior wall,lateral wall,inferior wall,interatrial septum myocardial during systolic and diastolic early,atrium contraction.Each period of peak strain rate(SRS、SRE、SRA)and maximum strain were measured.Results SRSof interatrial septum,inferior wall in LVH group compared with normal control group significant lower.Only SREof in the anterior wall in NLVH group significantly decreased compared with normal control and each wall of left atrial in LVH group significantly reduced compared with normal control group.SRAof inferior wall in LVH group was significant reduced compared with normal control group(*P,**P<respectively 0.05 and 0.01).Strain of each wall of left atrial in LVH group significantly reduced compared with normal control group(*P,**P<respectively 0.05 and 0.01).Strain in NLVH group no significantly difference compared with normal control.Conclusions Strain rate imaging can be objectively and accurately used to evaluate the left atrial function in patients with uremia.
