摘　　要：目的了解三级甲等医院医护人员为患者提供戒烟服务情况,为今后医院开展戒烟服务及制定相关控烟策略提供依据。方法随机抽取辽宁省3所三级甲等综合性医院一线医护人员(包括医生和护士)1021人,填写《医护人员戒烟服务调查问卷》。结果医护人员吸烟者比例占30.3%;主动劝告患者戒烟的仅占50.1%。医护人员劝告患者戒烟的影响因素有职称(初级:OR=3.179,95%CI:1.701~4.952;中级:OR=0.229,95%CI:0.138~0.405;高级:OR=0.357,95%CI:0.254~0.486)、科室(外科:OR=0.897,95%CI:0.767~0.906;其他:OR=0.912,95%CI:0.877~0.998)、知识得分(OR=0.776,95%CI:0.556~0.813)、医护人员(OR=3.198,95%CI:1.901~5.032)和是否吸烟(OR=0.906,95%CI:0.856~0.978)。结论医护人员吸烟比例较高,劝告患者戒烟率低;加大对医护人员的培训力度,特别是要增加控烟知识、控烟技能和控烟政策培训;降低医护人员自身的吸烟比例。Objective To understand the smoking cessation services provided by health care workers in tertiary level first-class hospitals,and provide evidence for smoking cessation services and relevant control strategies. Methods Totally 1021 medical doctors and nurses from 3 hospitals were surveyed by random sampling,and a questionnaire was used for information collection. Results Smoking rate of health care workers was 30. 3%. The rate of advising patients to quit smoking was 50. 1%. Influence factors of health care workers advising patients to stop smoking were as follows: technique title( junior titles: OR = 3. 179,95% CI: 1. 701- 4. 952; intermediate: OR = 0. 229,95% CI: 0. 138- 0. 405; senior: OR= 0. 357,95% CI: 0. 254- 0. 486), departments( surgery: OR = 0. 897,95% CI: 0. 767- 0. 906; other: OR =0. 912,95% CI: 0. 877- 0. 998),the score of knowledge( OR = 0. 776,95% CI: 0. 556- 0. 813),the medical staff( OR = 3. 198,95% CI: 1. 901- 5. 032) and smoking( OR = 0. 906,95% CI: 0. 856- 0. 978). Conclusion Smoking rate of health care workers was higher,while the rate of advising patients to quit smoking was low. Training,especially to increase the knowledge of tobacco control,technique skills and policy in health care workers should be taken. Smoking rate of health care workers should be reduced.
