头部自重后伸牵引治疗颈椎曲度异常的临床价值

摘　　要：背景:目前针对颈椎生理曲度异常的治疗方法各异,治疗效果差异很大,且不便于患者长期坚持。目的:探讨床旁头部自重后伸牵引治疗颈椎生理曲度异常的临床价值。方法:选择2016年3月至11月接受治疗的颈椎生理曲度异常患者60例,利用床旁头部自重后伸牵引的方法治疗4周。治疗前后均行颈椎侧位X线检查,测量曲度值,评价颈椎生理曲度恢复情况。记录并比较治疗前后疼痛视觉模拟评分(VAS)。结果:治疗前,颈椎反弓3例,颈椎生理曲度消失8例,颈椎生理曲度变直49例。治疗后,颈椎生理曲度恢复正常47例,变直13例;明显改变9例,改变不显著4例。治愈率达78.3%(47/60),总改变率达93.3%(56/60)。治疗后患者颈椎生理曲度值增加[(9.6±2.8)mm vs(2.3±1.8)mm],疼痛VAS评分降低[(0.7±0.5)分vs(4.2±1.3)分],且差异均有统计学意义(t=-26.68、28.51,P均<0.01)。结论:利用床旁头部自重后伸牵引治疗颈椎生理曲度异常疗效显著,是一种安全、有效的治疗方法。Background:There are varied treatment methods for abnormal physiological curvature of cervical vertebrae,and therapeutic effects vary greatly.Moreover,it is not easy for patients to persist for a long time.Objective:To explore the clinical value of bedside head self weight traction in the treatment of abnormal cervical curvature.Methods:Sixty patients with abnormal cervical curvature underwent bedside head self weight traction for 4 weeks between March 2016 and November 2016.Xray radiography of the lateral cervical spine was performed before and after treatment.The curvature of the cervical spine was measured.Visual analogue scale(VAS)score was recorded.Results:Before treatment,there were 3 cases of cervical back arch,8 cases of curvature disappearance and 49 cases of straightening.After treatment,cervical curvature became normal in47 cases,straight in 13 cases;changes were obviously found in 9 cases and not significantly in 4 cases.The cure rate was 78.3%(47/60),and the total change rate of cervical curvature was 93.3%(56/60).Compared with those before treatment,cervical curvature was significantly increased,while VAS score was significantly decreased after treatment([9.6±2.8]mm vs[2.3±1.8]mm,t=-26.68;0.7±0.5 vs 4.2±1.3,t=28.51,P<0.01).Conclusions:Bedside head self weight traction is a safe and effective method for abnormal cervical curvature.
