全膝关节置换术中吸引头细菌污染率的对照研究

摘　　要：目的调查全膝关节置换术(TKA)中吸引头细菌的污染率。方法对2015年2月至2015年5月在广州医科大学附属第一医院手术室行TKA手术的患者43例52膝进行对照研究,使用两套吸引器负压装置,在无菌区吸空气的吸引管头为对照组,手术中常规使用的吸引管头为实验组。对术中使用的吸引管头进行拭子细菌培养,统计细菌培养的阳性率。结果对照组术中1 h及术后吸引头拭子培养无菌群生长;实验组中有2例在术中1 h、术毕均培养出相同的表皮葡萄球菌,污染率为3.8%。两组比较无统计学意义(2=2.039,P>0.05)。结论在本研究中,TKA术中吸引头细菌污染率低,建议不需要强制常规每60分钟更换吸引管头。Objective To investigate the bacterial contamination rates of suction tip in total knee arthroplasty ( TKA).Methods Fifty-two knees undergone TKA in the operating room of the First Affiliated Hospital of Guangzhou Medical University from Feb 2015 to May 2015 were enrolled in this controlled study.Two sets of suction tubes were used in every TKA procedure, of which one was used in the control group without contacting the surgical field and the other was used routinely in the experimental group.Specimens were taken by swabs and sent for culture from inner wall of the suction tips one hour after the incision and at the end of the surgery.The bacterial contamination rates of the suction tip were compared between the two groups.Results In the control group, no flora was found in the laboratory.In the experimental group, the samples from the two suction tips presented positive culture results with identical staphylococcus epidermidis both one hour after the incision and at the end of TKA. The contamination rate of the experimental group was 3.8%, not significantly higher than the control group ( x2=2.039, P >0.05).Conclusion As the low contamination rate of suction tip in TKA, it is not suggested suction tip changing in every 60 minutes as a compulsory sterile protocol in TKA.
