封闭式负压引流术治疗手足皮肤撕脱伤的体会

摘　　要：目的探讨手足部皮肤撕脱伤患者应用封闭式负压引流方式(VSD)进行治疗的临床效果。方法所分析病例均来自本院2014年3月-2016年6月期间收治的手足皮肤撕脱伤患者,共58例,所有患者均采取断层皮片原位回植联合VSD治疗,随访时间3个月。结果患者用VSD治疗后可有效防止感染及皮片下血肿形成,促进皮片成活,减轻皮片收缩,治愈率为94.8%(55/58),有3例患者皮片发现有少许渗液,少许植皮坏死,经换药处理后痊愈;对患者进行3个月随访,其皮片颜色有所加深,皮片无明显收缩,手足功能恢复良好。结论对手足撕脱伤患者在常规采取断层皮片原位回植联合VSD治疗,充分引流有利于防止感染及皮片下血肿形成,促进皮片成活,减轻皮片收缩,可更好地促进手足功能的恢复,可作为当前对手足部皮肤撕脱伤治疗的主要手段。Objective To explore the clinical effect of closed negative pressure drainage(VSD)in the treatment to the skin avulsion of hand and foot.Methods All the 58 skin avulsion cases are from our hospital during March 2014-June 2016,who were treated with avulsed skin replantation in situ combined with VSD.The average follow-up time were 3 months.Results Patients with VSD treatment can effectively prevent the infection and hematoma under the skin graft,promote skin graft survival,reduce skin contraction.The cure rate was 94.8%(55/58).3 cases of patients with a little exudate under the skin graft and skin necrosis healed by wound dressing. After 3 months of follow-up,the color of the skin graft was deepened,the skin graft was not significantly contracted,and the function of the hands and feet recovered well.Conclusion The patients with hands and feet avulsion were treated with avulsed skin replantation in situ combined with VSD,could prevent the infection and hematoma under the skin graft,promote skin graft survival,reduce skin contraction,promote the function recovery of hands and feet.It can be used as the main treatment for the patient with hand and foot avulsion.
