尿四样蛋白联合检测对2型糖尿病肾病的早期诊断的价值研究

摘　　要：目的针对2型糖尿病肾病的早期诊断,探讨以尿四样蛋白联合展开检测的临床价值。方法选择120例2型糖尿病患者和40例健康体检人员的临床资料为研究对象,根据尿微量蛋白的排泄率将120例分为糖尿病早期肾病的观察组,将40例健康体检人员资料设置为对照组。对两组展开检测并比较微量白蛋白、α1微球蛋白、转铁蛋白、免疫球蛋白的含量,对所得结果进行分析并探讨其诊断价值。结果观察组的MA、AIM、IgU、TRU检测水平值比对照组的更高,对其展开统计学分析(P<0.05),差异具有统计学意义。MA+AIM+IgU+TRU的灵敏度、阳性似然比、阳性预测值均高于对尿蛋白四项单次检测,尿四样蛋白联合检测的特异性相对单独检测低,但其值也占有90.00%。结论对2型糖尿病肾病患者在早期诊断中以尿四样蛋白展开联合检测有很好的临床检测价值。Objective To investigate the clinical value of combined detection of urinary four protein in the early diagnosis of type 2 diabetic nephropathy.Methods The clinical data of 120 cases of patients with type 2 diabetes and 40 healthy people were selected as the research object,according to urinary albumin excretion rate of 120 cases of early diabetic nephropathy patients were divided into observation group,40 cases of healthy persons as the control group data set.The two groups were detected and compared the content of albumin,alpha 1 microspheres,transferrin and immunoglobulin.The results were analyzed and the diagnostic value was discussed.Results The levels of MA,AIM,IgU and TRU in the observation group were higher than those in the control group,and the statistical analysis was carried out(P<0.05),the difference was statistically significant.The sensitivity,positive likelihood ratio and positive predictive value of MA + AIM + IgU + TRU were higher than those of four single tests of urinary protein.The specificity of combined detection of urine four protein was relatively low,but its value was 90%.Conclusion In the early diagnosis of type 2 diabetic nephropathy,urinary four protein combined detection has a very good clinical value.
