高龄患者股骨颈骨折人工关节置换的临床疗效分析

摘　　要：目的分析人工关节置换对高龄股骨颈骨折患者的临床疗效及应用价值。方法选取我院2012年1月至2017年1月收治的330例股骨颈骨折患者,分为观察组与对照组。对照组患者均采用AO加压空心螺钉内固定治疗;观察组患者均采用人工关节置换治疗,分析并对比两组各项手术指标、临床疗效、康复效果以及生活质量的差异。结果经不同手术方式治疗后,观察组在手术时间、术中出血量及住院时间等项目上明显多于对照组,组间差异有统计学意义(P<0.05);观察组术后半年时间Harris评分明显高于对照组,组间差异有统计学意义(P<0.05);术后6个月,观察组在关节功能优良性上优于对照组(P<0.05),在并发症发生率上与对照组无统计学差异(P>0.05)。结论在对老年股骨颈骨折患者进行临床治疗时,人工关节置换术能显著改善患者术后关节功能及生活质量,且治疗更为安全,并发症发生率较低,有临床应用的价值。Objective To analyze the clinical efficacy and application value of artificial joint replacement in elderly patients with femoral neck fracture. Methods 330 cases of femoral neck fracture treated in our hospital from January 2012 to January 2017 were selected and divided into observation group and control group. The control group were treated with AO cannulated screw internal fixation treatment; the observation group was treated with artificial joint replacement treatment, analysis and comparison of two groups of the operation indexes, clinical efficacy, rehabilitation effect and quality of life differences. Results After different surgical treatments, the observation group in the operation time, bleeding volume and hospitalization time and so on significantly more than the control group,significant difference between groups(P<0.05); the patients in the observation group after six months Harris score significantly higher than the control group, significant difference between groups(P<0.05); 6 months postoperatively. After the observation group in the joint function excellent than the control group(P<0.05), the incidence of complications had no significant difference with control group(P>0.05). Conclusion The clinical treatment in patients of elderly femoral neck fracture, arthroplasty can joint function and life quality of patients was improved significantly after treatment, and more safety, low incidence of complications, clinical application value.
