脉压与冠心病患病关系的探讨

摘　　要：目的以横断面调查中老年人群血压水平与冠心病患病危险的关系,探讨收缩压、舒张压和脉压作为危险因素在冠心病患病危险上的差别,分析脉压对冠心病患病的影响。方法2001年调查驻京中老年(40～91岁)2 462名的血压水平和冠心病患病危险进行单因素和多因素logistic回归分析。结果 中老年人冠心病患病危险和收缩压与脉压呈正相关,与舒张压无关;单因素分析将收缩压水平分为5个亚组,与收缩压<120 mmHg相比,收缩压120～129、130～139、140～159和≥160 mm Hg的冠心病患病危险分别增加1.54、2.43、4.35和5.31倍;将脉压水平分为4个亚组,与脉压<40mm Hg相比,脉压40～49、50～59和≥60 mm Hg的冠心病患病危险分别增加1.5、4.03和7.11倍;多因素分析,脉压与冠心病患病危险的比值比为1.01。结论高血压是冠心病最常见、最重要的危险因素之一,收缩压和脉压是预测中老年人冠心病患病危险的较好指标,而脉压是中老年人冠心病患病的独立危险因子。Objective To reanalysis the relationship between blood pressure levels and coronary heart disease (CHD) by a cross sectional survey in middle-old-aged people, and to reevaluate the CHD predicting value of systolic blood pressure (SBP), diastolic blood pressure (DBF) and pulse pressure (PP). Methods The blood pressure survey date of 2 462 middle and old aged army people was studied at 2001 year. The blood pressure level and other risk factors of CHD were analyzed by using logistic mono and multifactor regression model. Results The incidence of CHD correlated positively with SBP and PP other than DBP. While compared to SBP level less than 120 mm Hg, the estimated odds ratio (OR) of SBP levels an 120 - 129,130 - 139,140 - 159 and over 160 mm Hg for predicting CHD were increased by 1.54,2.43,4.35 and 5.31 times individually. The OR of PP levels at 40 - 49,50 - 59 and over 60 mm Hg were increased by 1.5, 4.03, and 7.11 times while compared to under 40 mm Hg group. The OR of PP to predicting CHD was 1.01 in multifactor regression model. Conclusion Hypertension is one of the most common and important risk factor of CHD. The SBP and PP are the best predictors than DBP or MAP. The PP is the independent risk factor of middle and old aged CHD.
