摘  要:目的 了解新西兰兔血游离三碘甲状腺原氨酸(FT3),游离四碘甲状腺原氨酸(FT4)和促甲状腺素(TSH)参考值范围及兔氢氟酸皮肤烧伤后甲状腺功能的变化规律。方法 24只新西兰兔随机取15只用55%氢氟酸烧伤染毒5%体表面积。伤前5 min,伤后2、8、24、72、168、336 h分别采集静脉血3 ml,采用化学发光法检测FL3、FT4、和TSH。结果 ①兔FT3,FT4、和。TSH数值为偏态分布且方差不齐,经数据转换后采用K因子客许区间法计算得出参考值范围。②兔血FT3, FT4于伤后逐步下降,均于伤后24 h降至最低点(P<0.0001),FT3于伤后8 h下降差异即有显著性(P<0.05),FT4下降稍迟,于伤后24 h差异有极显著性(P<0.0001),随后血FT3、FT4浓度水平逐步升高,于伤后72 h至伤前水平,伤后168 h升高趋于平稳,与伤前相比差异仍有极显著性(P<0.0001)。TSH于伤后逐步下降,伤后24 h降至最低点(P<0.0001),随后TSH浓度水平逐步升高,于伤后72 h至伤前水平,之后TSH再次下降,于伤后168 h降低趋于平稳,与伤前相比差异仍有非常显著性(P<0.001)。结论新西兰兔氢氟酸皮肤烧伤中毒后甲状腺功能随病程的延长而逐步变化,对于临床救治氢氟酸烧伤病人有一定参考价值。Objective To evaluate normal limitation of free triiodothyronine(FT3), free thyroxine(FT4) and thyrotropin(TSH) in rabbits. And to investigate the changes of thyroid function in rabbits with hydrofluoric acid burn. Methods 15 New Zealand white rabbits were randomly taken from 24 rabbits and burned 5% body surface area by 55% hydrofluoric acid. Samples of blood were harvested at preburn and different postburn for the analysis. Results ① The normal limitation of the FT3,FT4 and TSH were showed in 24 rabbits by using K factor allowable arrange. ② The levels of FT3,FT4 decreased gradually after the burning and come to the lowest point at 24 h (P<0.0001). FT3 significantly reduced at 8 h (P<0.05) and FT4 significantly reduced at 24 h (P<0.0001) after the burning. Then FT3,FT4 increased gradually and recoverd to the normal levels at 72 h and come to the steady at 168 h after the burning which were significantly different from the levels before the burning (P<0.0001). The level of TSH decreased gradually after the burning and come to the lowest point at 24 h (P< 0.0001) and recoverd to the normal levels at 72 h, then TSH decreased once more and come to the steady at 168 h after the burning which was significantly different from the levels before the burning (P<0.001). Conclusion This study shows the changes of thyroid function after hydrofluoric acid burn in rabbits, which might provides some reference for the treatment of patients on clinic.
