摘 要:目的探讨2型糖尿病(T2DM)患者血糖控制不同阶段的血脂水平特点。方法对361例T2DM住院患者(根据既往病史及血糖水平分为血糖控制达标组、血糖控制较差组)及239例健康体检对照组分别测糖化血红蛋白(Hb A1c)、血清甘油三酯(TG)、总胆固醇(TCH)、高密度脂蛋白(HDL-C)及低密度脂蛋白(LDL-C)水平,比较血糖控制达标组、血糖控制较差组及对照组间血脂水平。结果与正常对照组比较,T2DM组TG、TCH、LDL-C水平明显升高,HDL-C水平降低;血糖控制达标组TCH、HDL-C和LDL-C水平无显著差异(P>0.05);而血糖控制较差组TCH和LDL-C水平与正常对照组比较显著升高,HDL-C水平降低(P<0.05);所有DM组的TG水平均与正常对照组有显著差异(P<0.05);TG、TCH、HDL-C和LDL-C均与Hb A1c具有平行关系(P<0.05),TG、TCH和LDL-C随Hb A1c升高而升高,HDL-C则相反。结论 T2DM血糖控制情况与血脂水平具有一定的关联性,血糖控制达标有助于TCH和LDL-C的降低及HDL-C的提高,对TG异常亦有所改善。Objective To investigate the characteristic of plasma lipid in different blood glucose control stages of type 2 diabetics( T2DM). Methods 390 T2 DM patients were divided into well-controlled and poorly-controlled groups in terms of medical history and blood glucose level,and 210 healthy samples were examined the levels of Hemoglobin A1C( Hb A1c),serum triglyceride( TG),total cholesterol( TCH),high density lipoprotein cholesterol( HDL-C) and low density lipoprotein cholesterol( LDL-C),respectively. Results After eliminating other factors on their influence of plasma lipid,compared with those of normal control group,the levels of TCH,HDL-C and LDL-C had no significant differences in the well-controlled group( P > 0. 05); while the levels of TCH and LDL-C were significantly higher and the level of HDL-C was decreased in poorly-controlled group( P < 0. 05). The level of TG had remarkable differences in T2 DM group( P < 0. 05),compared with that of normal control group. The levels of TG,TCH,HDL-C,LDL-C and Hb A1 c were parallel,the levels of TG,TCH and LDL-C were increased with the enhancement of Hb A1 c,while HDL-C was just the opposite. Conclusions Blood glucose control in T2 DM has a certain correlation with plasma lipid,controlling the blood glucose to target level helps TCH and LDL-C decrease and HDL-C increase,and improves TG anomalies.
