摘  要:目的 :探讨分娩前后纤维蛋白原 (Fb)浓度增加的机制。方法 :用分光光度计法检测Fb浓度 ;Fb片段的定量采用电泳和超声扫描仪 ;用免疫比浊法检测C反应蛋白 (CRP)。结果 :分娩前 ,总Fb及其片段浓度增加 ,CRP浓度正常。阴式分娩 3d后 ,总Fb浓度及其片段均比分娩前下降 (但仍明显高于对照组 ) ,其中高分子量Fb(HFb)下降程度比低分子量 (LFb)小 ,HFb/LFb比值增加 ;剖宫产后 3d ,总Fb浓度和HFb浓度升高 ,LFb浓度下降到与阴式分娩后相同水平 ,HFb/LFb比值增加 ;分娩后 3d ,两种分娩形式CRP浓度均增加 ,但阴式分娩者 ,浓度增加到分娩前的 3倍 ,剖宫产者浓度可增加到 2 5倍。分娩后 3d ,Fb或HFb浓度与CRP浓度呈显著相关。分娩 6周后 ,所有指标恢复正常。结论 :妊娠期间导致纤维蛋白浓度增高的机制是不同的。分娩后HFb/LFb比值增加依赖急性时相反应 。Objective: To study the mechanisms of thein crease of fibrinogen (Fb) during late pregnancy and after delivery. Met hods: Concentration of Fb was measured spectrophotometrically. Electrop horesis and densitometric scanning with ultrascan were used to determine the Fbfraction. C-reactive protein (CRP) concentration was determined by using immuno turbidmetric method. Results: Duringpregnancy,concentrationof total Fb and its fractions increased and that of CRP was normal. Three daysafter vaginal delivery, the concentration of total Fb and high molecular weightfibrinogen (HFb) decreased slightly and the ratio of high to low molecular weigh t fibrinogen (LFb) increased. After caesarean section, both of total Fb and HFband the ratio of HFb to LFb increased. The concentration of CRP increased aftereither type of delivery. The degree of increase, however, was 25 times as that a fter caesarean section. In all women at the end of puerperium, the concentration s of the compounds returned to normal. Conclusion: The mechani sms leading to the increase of Fb during pregnancy and after delivery are differ ent. The increase of ratio of HFb to LFb after delivery depends on the acute pha se reaction. The degree of this reaction depends on the type of delivery.
