阑尾肿物22例诊治探讨

摘　　要：目的 :通过22例阑尾肿物的追踪分析 ,探讨其诊断和外科治疗的疗效。方法 :回顾性研究1980～2002年接受治疗的 ,经过病理检查证实的阑尾肿物 ,根据其临床表现和辅助检查的特点 ,结合有关文献资料讨论阑尾肿物的诊治问题。结果 :粘液囊肿13例、类癌5例、腺瘤部份癌变1例、平滑肌肉瘤3例。术前仅有5例粘液囊肿获正确诊断。本组病例均行手术治疗 ,根据术中冰冻病检结果 ,3例类癌肿块直径>2cm者行右半结肠切除术 ,平滑肌肉瘤者行阑尾切除加盲肠部份切除 ,余16例仅行阑尾切除术 ,无1例死亡 ,均获良好疗效。结论 :对阑尾肿物术中应仔细探查和冰冻病检对正确的手术方式选择有重要作用。Objective:To study the definite diagnosis and surgical effect by tracing analysis of22cases of occuping mass in appendicular space.Methods:The occupying masses in appendicular space from1980～2002treated and confirmed by pathological examination in our hospital were studied retrospectively.Some problems of diagnosis and treatment of the occupying mass in appendicular space were discussed according to clinical manifestation and paramedical examination combined with the relevant literatures.Results:Of22cases with the occupying mass in apˉpendicular space,13were mucocele,5were carcinoid,1partial cancerization of adenoma and3leiomyosarcoma.Only5cases of mucocele were correctly diagnosed before operation.According to the results of refrigerated pathological examination,all of22cases were performed surgical treatment,3cases of carcinoid(diameter>2cm)were done for right hemicolectomy,3cases of leiomyosarcoma for appendectomy and partial ceˉcectomy,the other16cases only for appendectomy.No death was found,the curative effect in all cases was better.Conclusion:Intraoperative careful exploration and refrigerated pathological examination have important role on the choice of correct operation method.
