贝那普利降压疗效与校正QT间期的关系

摘　　要：目的　探讨原发性高血压患者心电图心率校正 QT间期 (QTc)和贝那普利降压疗效之间的关系。方法　采用前瞻性队列研究方法调查安徽 A县 899名高血压病患者的基线心电图、治疗1 5 d前后的血压变化及相关临床和流行病学特点等情况。结果　 QTc和血压下降呈负相关。 QTc每增加 0 .1秒 1 /2 ,收缩压和舒张压的下降值分别减少 3.6 m m Hg和 3.3m m Hg,且差异均有显著性(P <0 .0 5 )。将 QTc三等分后 ,随着 QTc等分的增加 ,收缩压和舒张压的下降值均逐渐减少 ,且差异具有显著性 (P <0 .0 5 ) ;将 QTc等分后的变量放入方程中进行趋势性分析 ,发现在收缩压和舒张压组中的负相关关系具有显著性 (P <0 .0 5 )。随着 QTc等级的增加 ,舒张压有效率逐渐降低 ,且有显著性差异 (P <0 .0 5 ) ,收缩压的效应关系不如舒张压明显。结论　 QTc和贝那普利降压疗效呈负相关 ,对贝那普利短期降压疗效具有重要的预测价值。Objective To evaluate the relationship between the heart rate corrected QT interval (QTc) and anti hypertensive efficacy response to benazepril treatment in essential hypertension patients. Methods A prospective cohort study was carried out to investigate baseline ECG, blood pressure changes before and after the treatment, clinical features and epidemiological information during treatment among 899 essential hypertension patients in county A of Anhui province, China. Results There was significantly negative correlation between QTc and the blood pressure drop(P<0.05). 0.1 second 1/2 increase of QTc was associated with 3.6 mmHg and 3.3 mmHg decrease in SBP and DBP, respectively. When equally divided QTc into three portions, the values of blood pressure drop were appreciably attenuated with increase of QTc portions(P<0.05). The significantly negative correlation between QTc and ΔSBP or ΔDBP were also observed after trend analysis. The effective rate of ΔDBP, not ΔSBP markedly dropped followed by the increase of QTc portions. Conclusions There were generally negative correlation between the QTc interval and blood pressure drop response to Benazepril treatment, baseline QTc interval may be useful in predicting short term benazepril responses.
