彩超对胎儿脐带缠绕肢体的产前诊断价值

摘　　要：目的 :探讨胎儿脐带缠绕肢体的彩超声像图表现及其对临床的研究价值。方法 :10 0 0例孕妇行彩超产前常规检查 ,对 2 4例存在脐带缠绕肢体的超声声像图表现进行研究。结果 :彩超诊断胎儿脐带缠绕体的准确率 99 6% ,敏感性 95 2 4% ,特异性 99 69% ,且胎儿脐带缠绕肢体合并其他部位缠绕组胎儿的死亡率、剖宫产率、胎儿宫内窘迫的发生率均明显高于单纯缠绕肢体组 (P <0 0 5 )。结论 :脐带缠绕肢体具有特征性超声声像图表现 ,可作为常规产前检查项目 ,对于早期诊断、确定胎儿的分娩方式 。Objective:Discuss the colored ultrasonic image manifestation of fetus umbilical cord enwinding limbs and the research value on clinic.Methods:Among 1 000 cases of pregnant women who accept antenatal routine inspection by colored ultrasonography,there are 24 cases that have umbilical cord enwinding limbs and we′ll study the ultrasonic image manifestation of them.Results:The rate of accuracy that has been diagnosed as fetus umbilical cord enwinding limbs by colored ultrasonography is 99 6%,sensitivity is 95 24% and specificity is 99 69%.At the same time the death rate,the rate of cesarean birth and the occurrence rate of fetus distress in uterus that arise by fetus umbilical cord incorporating with other parts enwinding limbs is obviously higher than that of arise by simply enwinding limbs groups( P <0 05).Conclusion:As fetus umbilical cord enwinding limbs has the characteristic quality ultrasonic image manifestation,colored ultrasonic inspection can be used as one of the antenatal routine inspections and has important clinical value in diagnosing in the early stage,confirming the fetus′ childbirth way and improving the birth quality of perinato pause.
