摘  要:目的:观察辛伐他汀对一氧化氮(nitricoxide,NO)缺乏性高血压大鼠肾功能的保护作用,并探讨其机制和可能的干预方法。方法:L-硝基精氨酸甲酯(NG-nitro-L-argintnemethylester,L-NAME造成NO缺乏性高血压大鼠模型,随机分为对照组、高血压组、辛伐他汀组,每组8只。测定尿β2-微球蛋白(β2-M)、尿蛋白,肾脏电镜图及NO和血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)等水平。结果:给予L-NAME2周始血压持续升高,第7天和8周时尿蛋白犤(40.5±10.5)mg/L和(48.4±5.6)mg/L犦及β2-M犤(0.58±0.11)mg/L和(0.64±0.04)mg/L犦形成两个高峰,8周时肾小球滤过膜明显损伤;辛伐他汀在不影响血压和血脂水平条件下,减少尿蛋白和尿β2-M排出;减轻肾小球滤过膜损伤,肾脏局部NO和AngⅡ水平发生显著的差别。结论:辛伐他汀对NO缺乏性高血压大鼠早期肾功能损害有保护作用,其机制可能与影响肾脏局部内皮功能和AngⅡ水平有关。AIM: To observe the protective effect of simvastatin on renal function in rats with nitric oxide(NO) deficient hypertension, and to explore its mechanism and possible intervention. METHODS:The models of rat with NO deficient hypertension were made by using NG nitro L arginine methyl ester(L NAME), then the rats were randomly divided into control group(n=8), hypertension group(n=8) and simvastatin group(n=8).Urinary protein and β 2 microglobulin protein(Uβ 2 M) were measured, the renal graph was analyzed with electron microscope, and the levels of NO and angiotensinⅡ (AngⅡ ) were measured. RESULTS:Blood pressure of the rats began to increase 2 weeks after L NAME was given, the urinary protein[(40.5± 10.5) mg/L and ( 48.4± 5.6) mg/L] and Uβ 2 M [(0.58± 0.11)μ g/L and (0.64± 0.04) μ g/L]achieved the peak at the 7th day and the 8th week, and the glomerular filtration membrane was damaged obviously at the 8th week.Simvastatin decreased the discharging of urinary protein and Uβ 2 M under the condition of no effect on the levels of blood pressure and lipid,and reduced the glomerular filtration membrane damage.There were significant differences between the local renal NO and AngⅡ level in renal tissue. CONCLUSIONS:Simvastatin has a protective effect on early renal function damage in rats with NO deficient hypertension.Its mechanism may be related to the effect on the endothelial function and the AngⅡ level in local renal tissue.
