摘 要:背景:心肌挫伤后心功能障碍除与心脏因受暴力直接损害导致心肌收缩性降低外,是否还与心肌细胞内Ca2+和线粒体ATP酶的变化有关目前尚不得而知。目的:探讨心肌细胞内游离钙(犤Ca2+犦i)和心肌ATP酶变化在心肌挫伤后心功能障碍发生机制中的意义。设计:随机对照的实验研究。地点、材料和干预:实验在解放军第三军医大学野战外科研究所完成。选取四川健康家兔36只,雌雄不限,分在伤前,伤后2,4,8,12,24h组,每组6只。经右颈总动脉插管至左心室。采用BIM-Ⅱ型生物撞击机致成心肌挫伤模型。主要观察指标:心肌挫伤前,伤后2,4,8,12,24h测定家兔左室收缩/舒张功能、心肌细胞内犤Ca2+犦i含量和心肌匀浆组织及线粒体ATP酶活力。结果:左心室收缩/舒张功能受到损害,代表左心室收缩功能的左室收缩末压(leftventricularend-systolicpressure,LVESP)、左室内压最大上升速率(themaximalrateofleftintraventricularpressurechanges,+dp/dtmax)、等容收缩压(isovolemecventricularpressure,IP)、实测心肌最大收缩速度(themaximalphysiologicalvelocity,Vpm)在伤后4~12h内恢复至伤前水平(P>0.05);代表左心室舒张功能的舒张期左室内压下降时间常数T、左室舒张末压(leftventricularend-diastolicpressure,LVEDP)、左室内压最大下降?BACKGROUND:It is still unknown that whether cardiac dysfunction is related with intramyocardial calcium and myocardial ATPase except mycardial systolic pressure decreases induced by direct violent damages after myocardial contusion. OBJECTIVE:To explore the mechanic significance of intramyocardial calcium and myocardial ATPase in cardiac dysfunction after myocardial contusion(MC) in rabbits. DESIGN:A randomly case control study. SETTING,MATERIALS AND INTERVENTIONS:The research was completed in Third Military Medical University.36 Sichuan healthy rabbits selected with either gender were allocated into before injury,2,4,8,12,and 24 hours after injury groups with 6 rabbits each.Cannula was inserted into left ventricule through right common carotid artery.The models of MC were established with BIM Ⅱ bio impactor. MAIN OUTCOME MEASURES:Cardiac functions,free calcium content and the ATPase activity in myocardial tissue and cardiomyocytic mitochondria before injury 2,4,6,8,12 and 24 hours after injury. RESULTS:The function of contraction/diastolization of left ventricular was impaired.Indices represented left ventricular contraction function were left intraventricular end systolic pressure(LVESP),the maximal rate of left ventricular pressure change(+ dp/dtmax),isovolumetric ventricular pressure(IP),and the maximal physiological velocity(Vmp),which returned to the level before MC in 4 to 12 hours after MC(P >0.05);main indices represented left ventricular diastolization function were descending time constant T of left intraventricular pressure during isovolumetric diastolization phase,left ventricular end diastolic pressure(LVEDP),and the maximal rate of left intraventricular pressure changes(- dp/dtmax),which had significant differnce compared between before MC and 24 hour after MC(P< 0.05,P< 0.01). The average value of fluorescence of intramyocardial Ca2+ before MC was (2.26± 0.16),and began to increase to (2.63± 0.4) after 2 hours of MC (P >0.05),and hit its peak value of (3.56± 0.33) at 8 hour a
