摘 要:目的 :探讨孕晚期脂肪细胞膜脂肪酸组成与胰岛素抵抗的关系。方法 :正常妊娠 40例 ,妊娠期糖尿病 (GDM)孕妇 41例 ,以胰岛素敏感指数 (ISI)评价胰岛素抵抗程度 ;在剖宫产时随机抽取正常妊娠和GDM孕妇各 1 0例 ,取皮下脂肪各 1 0g,用气相色谱法测定脂肪细胞膜脂肪酸。结果 :GDM孕妇ISI为 4 .2 5± 0 .51 ,明显高于正常孕妇的 3.52±0 .56,(P <0 .0 0 1 )。GDM脂肪细胞膜饱和脂肪酸 (SFA)百分含量为 30 .42±1 .1 6,高于正常妊娠的 2 6 .92±3 .63(P <0 .0 5) ;多不饱和脂肪酸 (PUFA)百分含量为 2 5 .94± 2 .82 ,明显低于正常妊娠的 30 .2 7± 3.2 1 (P <0 .0 1 ) ,单不饱和脂肪酸 (MUFA)百分含量为 43.64± 3 .31 ,正常妊娠为 42 .81± 1 .80 ,差异无显著性 (P >0 .0 5)。对ISI与脂肪细胞膜脂肪酸饱和程度偏相关分析 ,ISI与脂肪细胞膜SFA含量呈正相关 (r=0 .5541 ,P <0 .0 5) ;与脂肪细胞膜PUFA含量呈负相关 (r=- 0 .52 93 ,P <0 .0 5) ;与脂肪细胞膜MUFA无相关。结论Objective:To study the relationship between the insulin r esistance and membrane fatty acids in adipocyte during late pregnancy. Methods:40 normal gravida (control group) and 41 gravida with GDM (study group) were recruited. The degree of insulin resistance was evaluated by insulin sensitivity index (ISI).Membrane fatty acids in adipocyte was analyz ed by gas chromatograph in 10 pregnant women respectively. Results: ISI in study group was 4.32±0.51, higher than 3.52±0.56 in control group(P<0.001).Membrane saturated fatty acids (SFA) content (as % by weight of total fatty acids) in study group was 30.42±1.16,higher than that in contro l group (26.92±3.64)(P<0.05);Polyunsaturated fatty aci ds (PUFA) content (25.94±2.82)lower than that in control group (30.27±3.2 1)(P<0.01) in adipocyte respectively. Monounsaturated fatty acids ( MUFA) content did not differ significantly between pregnant women with GDM and n ormal pregnant women. Co rrelation analysis demonstrated a significantly positive correlation between ISI and SFA content(r=0.5541,P<0.05).PUFA content was significantly negatively relate d to ISI(r=-0.5293, P<0.05), but MUFA content was not correlated with ISI.Conclusion: It's likely that elevation of insulin resistance in women with GDM may be relate d to low content of membrane PUFA in adipocyte.
