摘  要:目的:探讨细胞间黏附分子-1(ICAM-1)在大鼠脊髓缺血再灌注损伤(I/R)后表达变化及其意义。方法:采用Zinen法建立脊髓缺血再灌注损伤模型。应用半定量RT-PCR方法、免疫组化及免疫荧光激光共聚焦显微镜定性、定量测定脊髓微血管内皮细胞表面ICAM-1mRNA表达变化。脊髓组织髓过氧化物酶(MPO)活性测定用于定量反映浸润到脊髓组织中的多形核白细胞(PMN)数量。结果:正常脊髓微血管内皮细胞表面仅有微量的ICAM-1表达,组织中未见PMN浸润,单纯缺血不引起ICAM-1表达。再灌注后损伤区I-CAM-1表达及PMN的浸润先后发生了改变;再灌注4h,ICAM-1mRNA表达量明显升高,再灌注12h其在单位微血管面积上的荧光强度约比单纯缺血组增加了二分之一,再灌注9~12h,脊髓组织中MPO活性约为单纯缺血组的两倍。提示脊髓损伤后微血管内皮细胞初始ICAM-1表达增加是由再灌注损伤激发,其后延迟递增表达增强,并相伴有PMN向脊髓内浸润,说明ICAM-1参与了脊髓再灌注损伤的炎症病理过程。结论:ICAM-1可作为评判脊髓损伤程度和脊髓I/R后炎症反应程度的监测指标。AIM:To investigate the significance and change of the expression of microvascular intercellular adhesion molecule 1(ICAM 1) after spinal cord ischemia/reperfusion(I/R) injury. METHODS:The model of spinal cord I/R injury was established with the Zinen's method.The expression of vascular endotheliocytes ICAM 1mRNA in spinal cord injury was detected through the adoption of semi quantitative reverse transcription polymerase chain reaction(RT PCR),immunohistochemistry and immunofluorescence technique.The measurement of activity of myeloperoxidase(MPO) was used to quantificationally express the number of polymorphonuclear leukocytes(PMN) which was infiltrated into the spinal cord tissue. RESULTS:Less expression of ICM 1 was observed on the surface of microvessel endothelial cell in the spinal cord,no infiltration of PMN was found in the tissue,and the expression of ICAM 1 was not caused by pure ischemia.The expression of ICAM 1 in ischemia area after reperfusion and the infiltration of PMN changed successively.The expression of ICAM 1mRNA was increased remarkably four hours after reperfusion,and after reperfusion for twelve hours its fluorescent intensity on microvascular area per unit was increased about half more than that of the pure ischemia group.The activity of MPO in spinal cord issues was about twice as much as that of the pure ischmia group. CONCLUSION:The ICAM 1 can be as the monitoring marker of spinal injury degree and inflammatory reaction after spinal cord I/R injury.
