摘 要:目的 探讨氨茶碱与肾上腺素合用在大鼠窒息致心脏停搏中的疗效是否优于单纯使用肾上腺素或氨茶碱。方法 采用呼气末夹闭气管 8min ,建立大鼠窒息致心脏停搏模型。 36只大鼠随机分为氨茶碱 ( 10mg kg) +肾上腺素 ( 0 .0 2mg kg)组、肾上腺素 ( 0 .0 2mg kg)组和氨茶碱 ( 10mg kg)组 ,分别行心肺复苏 ,比较 3组大鼠的心脏电活动和自主循环恢复情况以及存活率。结果 心电活动恢复率 3组无显著性差异。起效时间氨茶碱 +肾上腺素组为( 4 3.0± 34.0 )s,氨茶碱组为 ( 39.0± 2 3.0 )s ,两组显著快于肾上腺素组 ( 90 .0± 4 2 .0 )s。自主循环恢复率 3组无显著性差异。自主循环恢复大鼠的平均血压氨茶碱 +肾上腺素组为 ( 5 5 .5± 17.3)mmHg( 1mmHg =0 .133kPa) ,显著高于肾上腺素组 ( 2 6 .7± 7.8)mmHg和氨茶碱组 ( 15 .0± 2 .4 )mmHg。 1h存活率 3组无显著性差异。结论 氨茶碱和肾上腺素合用在窒息致心脏停搏大鼠模型中 。Objective To compare the efficacy of aminophylline vs.epinephrine vs.the combination of aminophylline with epinephrine in treatment of a rat model of asphyxial cardiac arrest.Methods Asphyxial cardiac arrest was induced by clamping the endotracheal tube.After 8 min of cardiac arrest,a bolus dose of either aminophylline 10 mg/kg,epinephrine 0.02 mg/kg,or a combination of aminophylline 10 mg/kg with epinephrine 0.02 mg/kg was given to 3 groups respectively in a randomized manner.Results The rate of cardiac electrical activity restoration,spontaneous circulation recovery and 1 h survival rate were not statistically different between 3 groups.Initiating time of resuscitation was (43.0±34.0)s in aminophylline epinephrine group and (39.0±23.0) s in aminophylline group.Either group was significantly earlier than that in epinephrine group(90.0±42.0) s.The mean arterial pressure in those achieving recovery of spontaneous circulation was (55.5±17.3) mm Hg in aminophylline epinephrine group,which was significantly higher than that in epinephrine group(26.7±7.8) mm Hg and aminophylline group(15.0±2.4) mm Hg.Conclusion These results suggest that the combination of aminophylline with epinephrine can improve the recovery of blood pressure in the rat model of asphyxial cardiac arrest.
