摘  要:目的:根据精液中生精细胞的有无、形态和比例,进行精象分析,作为评价睾丸功能和鉴别阻塞性和非阻塞性无精子症的有效方法。方法:用精液涂片,瑞-吉氏染色,油镜下分类和分型,并以睾丸活检病理所见对照。结果:697例不育者中发现70例无精子,检出率为10.04%,有生精细胞者42例(60%),95.2%有精母细胞和精子细胞;无生精细胞者28例(40%),两者的睾丸大小显著不同。病因以服用过棉籽油占30.00%;原因不明占22.86%;腮腺炎史占12.8 6%;输精管因素占10.00%。精液细胞学检查与睾丸活检病理诊断基本一致。结论: 精液细胞学检查是一种无创伤性、无后遗症、无痛苦和操作简便,适用于基层判断睾丸功能和区别病因的有效的方法,值得推广。Objective To evaluate testicular function and differentiate between occlusive and non-occlusive azoospermia by analysis of presence, morphology and proportion of spermatogenic cells in the semen. Methods Semen smears were stained with wright-Giemsa's stain, examined under microscope for classification and typing of spermatogenic cells, and compared with histopathological findings in biopsy specimens of testes. Results Of 697 infertile male patients, azoospermia was found in 70, with a detection rate of 10.01 % , including42(60%) having spermatogenic cells, of whom 40 had spermatocytes and spermatids, and 28(40%) having no spermatogenic cells; the testes differed in size between the two groups. Etiologically, history of eating cottonseed oil accounted for 30.00% , unknown cause 22.86% , history of parotitis 12.86 % , and factor of vas deferens 10.00% . The results of seminal cytological examination were basically in agreement with the histopathological findings of testicular biopsy. Conclusion Seminal cytological examination is an atraumatic, painless, simple procedure without complications, and suitable for use in primary medical care establishments to evaluate testicular function and differentiate etiology of male infertility.
