电视宫腔镜输卵管插管注药术在不孕症诊治中的应用

摘　　要：目的　观察电视宫腔镜下输卵管插管加压注药术诊治输卵管梗阻及输卵管通而不畅的疗效。方法　选择经子宫输卵管碘油造影诊断为输卵管梗阻及输卵管通而不畅的不孕患者 112例 ,共 2 0 6条输卵管 ,经宫腔镜将输卵管间质部导管插入输卵管间质部约 0 .5 -0 .8mm ,注入美兰少许证实无误后 ,再注入药液。每月注药一次 ,连续 1-6月。结果　治疗后 15 7条输卵管通畅度改善 ( 76.2 % ) ,与治疗前有非常显著性差异 (P <0 .0 1) ,其中 99条输卵管通畅 ( 4 8.1% ) ,5 8条好转 ( 2 8.1% ) ,49条无效 ( 2 3 .8% ) ,此术用于输卵管通而不畅者疗效较佳 ( 89.4% )。另显示治疗次数越多疗效越好。治疗后输卵管通畅的 99条 5 0例中 ,随访 1-12月已有 3 0例获宫内妊娠 ,有 1例异位妊娠。结论　电视宫腔镜下输卵管插管加压注药是诊治部分输卵管梗阻的一种较安全。Objective This paper aims at the observation of the effect of the treatment of oviduct obstruction and oviduct passage problem with the method of medicine injection via oviduct cannula under TV hysteroscope.Methods We chose 112 cases of sterility patients with oviduct obstruction and oviduct passage problem as diagnosed using iodized oil radiography of womb oviduct,and we had 206 oviducts in total for our observation.We inserted oviduct mesenchyme cannula used into the oviduct mesenchyme for 0.5-0.8mm via hysteroscope,and injected small amount of methylene blue into it.After it was proved successful,medicine solution wan then injected.This procedure was done each month and lasted continuously for 1～6 months.Results The final result is that after the treatment the degree of unblockedness for 157 oviducts has improved (76.2%) and the effect due from the treatment is tremendous (P<0.01).Among these 157 oviducts,99 behaved unblocked (48.1%) and 58 improved (28.1%).Only 49 oviducts under the treatment turned out to be ineffective (23.8%).It demonstrates that the treatment method is pretty effective for patients with oviduct passage problem (89.4%),and the higher the number of treatment is,the more effective the treatment will be.Among the 48 cases with 99 oviducts that became unblocked after treatment,30 cases of uterine pregnancy and 1 case of heterotopic pregnancy were achieved after follow-up treatment for 1 to 12 months.Conclusions As a result,we can conclude that the treatment is a safe and effective method for treating at least prat of the oviduct obstruction cases.
