摘 要:目的 :探讨高氧液经静脉和消化道对高原低氧血症的作用 .方法 :选择首次进入高原的健康志愿者 (脉搏氧饱和度低于 90 % ,SaO2 <90 % ) 4 0名 ,随机分为静脉组 (V组 )和消化道组 (D组 ) ,其中每组根据海拔高度的不同 ,又分为 30 0 0m(V3 0 0 0 ,D3 0 0 0 )组和 4 5 0 0m (V450 0 ,D450 0 )组 .每组均按 6mL·kg-1给予高氧液 ,在不同的时间点监测动脉血气和心率的变化 ,同时观察使用的安全性 .结果 :所有受试者在应用高氧液之前 ,动脉氧分压均低于 8.0kPa.静脉和消化道两种给药途径间无差别 (P 0 .0 5 ) ;不同时间点之间差异有统计学意义 (P <0 .0 5 ) .V组在静脉输注高氧液 2 0min后 ,PaO2 开始上升 ,4 0min时作用达到高峰 ,随后随时间推移逐渐下降 .D组在口服高氧液 2 0min后 ,PaO2 开始上升 ,6 0min时作用达到高峰 ,随后逐渐下降 .动脉氧饱和度的变化是随着PaO2 升高而升高 ,而心率的变化则是随着PaO2 的升高而降低 .未观察到气体栓塞等不良反应 .结论 :高氧液经静脉和消化道途径 。AIM: To observe the therapeutic effects of hyperoxia solution orally or transvenously on hypoxemia in high altitude. METHODS: Forty healthy volunteers who were in high altitude region for the first time were randomly allocated into vein group (V) and digestive tract group (D). The volunteers of each group were divided further into two subgroups again (V 3000 , D 3000 and V 4500 , D 4500 ) and were given hyperoxia solution 6 mL·kg -1 transvenously or orally accordingly. Blood gas analysis and HR were measured at different time point and side effects were also observed. RESULTS: PaO 2 in all volunteers were lower than 8.0 kPa before the administration of hyperoxia solution. In group V, PaO 2 ascended gradually 20 min after transvenous infusion of hyperoxia solution and reached the peak after 40 min. Compared with that before infusion, there was no significant difference after 120 min. Likewise, in group D, PaO 2 increased 20 min after taking hyperoxia solution and reached the peak after 60 min, but there was still a significant difference after 120 min. The changes of SaO 2 had a positive correlation with PaO 2 and the changes of HR had a negative correlation with PaO 2. No side effects were observed. CONCLUSION: Hyperoxia solution orally or transvenously is simple, safe and effective for hypoxemia induced by high altitude.
