摘 要:目的 研究一氧化氮(NO)及一氧化氮抑制剂(L-NMMA)对腹膜透析通透性,特别是高糖透析中的作用。方法 建立大鼠长期腹膜透析模型,测定不同浓度透析液中以及不同刺激条件下腹膜巨噬细胞产生NO的水平。分别用1.5%、4.25%的葡萄糖透析液及分别加L-NMMA(5mg·kg^(-1)·d^(-1))行SD大鼠透析。10d后测定其腹膜动力学的变化。结果 腹膜巨噬细胞产生NO的水平,4.25%的葡萄糖透析液高于1.5%葡萄糖透析液(P<0.01),而后者又高于生理盐水(P<0.01)。高糖透析可减少腹膜的液体超滤(P<0.05)及溶质的转运。加用L-NMMA后腹膜的液体超滤及溶质的转运增加(P<0.05)。结论 腹膜透析时腹腔产生NO的水平增高,对腹膜的通透性增高中起一定作用。透析液中加入L-NMMA可改善腹膜的通透性。Objective To study the effect of nitric oxide (NO) and its inhibitor on the peritoneal permeability in rats during peritoneal dialysis,especially with high glucose dialysate. Methods The animal model of peritoneal dialysis was established and the levels of nitric oxide produced by peritoneal macrophages during peritoneal dialysis with different conditions were measured. The changes of peritoneal kinetics in rats after peritoneal dialysis with 1.5%,4. 25% dialysate and N: (G)-monomethyl-L-arginine(L-NMMA, 5 mg . kg -1. d -1) adding to the dialysate respectively for ten days were observed. Results The level of nitric oxide produced by macrophages during peritoneal dialysis with the 4. 25% dialysate was significantly higher than (hose with 1. 5% dialysate and normal saline( P < 0. 01) . The water ultrafiltration in the rats dialyzed with 4. 25% dialysate obviously decreased compared to that dialyzed with 1. 5% dialysate and that in the control group(P < 0. 05) . The water ultrafiltration and the solute transport significantly increased when adding L-NMMA to the dialysate( P < 0. 05) . Conclusions The level of NO increases in the rats during peritoneal dialysis. NO may affecte the peritoneum permeability and the nitric oxide inhibitor adding to the dialysate can improve the water ultrafiltration and the solute transport.
