螺旋CT三维重建技术在四肢骨关节骨折与脱位中的应用

摘  要:目的:探讨螺旋CT多平面重建(MPR)、三维表面遮盖法重建(SSD)及容积重建(VR)技术在四肢骨关节骨折与脱位中的临床应用价值。方法:对71例四肢骨关节骨折的患者进行螺旋CT容积扫描,在工作站上进行MPR、SSD、VR成像,结合轴位扫描图像对骨折分型,观察3种重建图像在显示骨折线、骨折碎片以及四肢关节脱位方向和程度的效果。结果:MPR对判断关节脱位方向和程度有帮助;SSD显示骨折和关节脱位的立体效果较好;VR虽然立体效果稍逊于SSD,但能显示骨折的细微结构。结论:MPR、SSD、VR3种重建方法可互为补充,能够立体直观、整体地显示四肢骨关节骨折和关节脱位,对临床医生决定治疗方案、患者预后的估计很有帮助。Objective:To investigate the clinical value of helical CT multip la nar reconstructions (MPR), surface shading display (SSD) and volume reconstructi ons(VR) in patients with fracture of extremital bone and joint. Methods:Seventy -one cases with fracture of extremital bone and joint were volumetrically scann ed by helical CT. After scanning, the raw data were transferred to workstation a nd processed with MPR, SSD and VR. According to the images of MPR, SSD, VR and c ombiding of transaxial images, the fractures and dislocations of extremital bone s and joints were displayed. Results:MPR images were of helpful to in diagnosis in the directions and degrees of dislocations of extremital joints. SSD can sho w the fractures and dislocations of extremital bones and joints stereoscopically . VR was more accurate in showing tiny fractures and elaborative structures, alt hough SSD was superior to VR in display of spatial connection. Conclusion:Combi nation of MPR, SSD and VR can gain an advantage from showing fractures and the d islocations of extremital bones and joints more accurately and stereoscopically. Those techniques have a great value in choosing therapeutic method and approach to surgery. [
