摘 要:目的 研究大鼠单纯胰腺移植 (PTA)的不同手术方法。方法 近交系SD大鼠为供受体 ,根据胰外分泌系统的不同处理 ,分为正常对照组 (NDC组 ,n =10 )、糖尿病大鼠对照组 (DC组 ,n =10 )、胰肠重建式 (E -D组 ,n =2 2 )和胰膀胱重建式 (B -D组 ,n =2 2 ) ,血管吻合采用供胰腺腹主动脉绊 (含腹腔动脉及脾动脉 )及门静脉绊 (含脾静脉 )与受体腹主动脉及左肾静脉分别行端侧和端端吻合 (袖套式 ) ;记录手术时间、术后死亡时间及原因 ,并于术前和术后 1、3、7、14、30天监测受体体重、饮食量、饮水量和空腹血糖。结果 两组手术难度上无差异 ;E -D组除 11例 (5 0 % )于 1月内死亡或胰腺失去功能外 ,均成活 1月以上 ;并发症主要为胰漏及继发性胰腺炎 (7例 ,31.8% ) ;B -D组除 12例 (5 4 .5 % )于 1月内死亡或胰腺失去功能外 ,均成活 1月以上 ;并发症主要为尿路并发症 (7例 ,31.8% ) ;成功的胰腺移植后第 1天血糖即下降 ,饮水、食量、尿量均减少 ,第 7天基本达到正常水平。结论 成功的E -D和B -D重建式PTA均可有效地恢复糖尿病大鼠内分泌功能 ;如何防治术后并发症是移植成功的关键。Objective:To investigate two different ways to establish the models of the rat's pancreas transplantation alone (PTA). Methods:Inbred SD rats were randomly assigned to 4 groups: Group NDC consisted of 10 nondiabetis control rats, Group DC consisted of 10 diabetis rats without PTA, Group E-D consisted of 22 diabetis rats received PTA with the enteric drainage, and group B-D consisted of 22 diabetis rats received PTA with bladder drainage. Venous drainage of PTA was performed end-to-side anastomosis between the donors' abdominal aorta and recipients' abdominal aorta segment, and end-to-end anastomosis between the donors' portal vein segment and recipients' left renal vein(use a cuff). The blood glucose, body weight, food intake, water intake and urine volume were monitored before transplantation and on the 1st, 3rd, 7th, 14th, 30th day after it, simultaneously the reasons of the failure were analyzed.Results: The operative time of recipient in E-D group was significantly longer than that in B-D group. 11(group E-D, 50%) and 12(group B-D, 54.5%) cases died or lost their function of the endocrine within 1 month.The main complications of the group E-D were followed pancreatitis and postoperation pancreas leak (7 cases, 31.8%), those of the group B-D were urinary complications(7 cases, 31.8%). In the two groups, all successful recipients' blood glucose lowered on the first day after transplantation, and their food intake, water intake and urine volume decreased and became stable 14 days later.Conclusions:Suc-cessful PTA with the E-D and B-D can recover the pancreatic endocrine function of the diabetic rats.The key of the successful transplantation was to prevent the posttransplantation complications.
