长沙地区女性胫骨超声速率随年龄的变化

摘　　要：目的　横断面调查长沙地区女性胫骨超声速率 (SOU)随年龄的变化 ,建立定量超声 (QUS)诊断骨质疏松 (OP)参考数据库。方法　采用SoundScan 2 0 0 0型QUS仪测量 15 96例 12～ 96岁女性右侧胫骨中点SOU。结果　按每 5岁年龄分组分析的结果显示 ,胫骨SOU的峰值为 ( 3 991.4± 68.0 )m/s ,出现于 3 5～3 9岁 ;3 5岁前SOU随年龄增加而增加 ,平均每 5年增长几何均数 (G)为 1.41% ;40岁后SOU随年龄增加而下降 ,平均每 5年下降G为 1.0 7%。SOU随年龄的变化曲线用三次回归分析模型拟合优度最佳 (R2 =0 .5 0 5 ,P <0 .0 0 1) ,其三次方程为SOU =3 3 83 +3 9.9× (年龄 ) -0 .78× (年龄 ) 2 +0 .0 0 3 9× (年龄 ) 3 ;胫骨SOU达到OP诊断标准的发生率是 :40～ 49岁为 4.7% ,5 0～ 5 9岁为 2 7.0 % ,60～ 69岁为 62 .2 % ,70～ 79岁为 79.6% ,≥ 80岁为 89.7%。结论　女性胫骨SOU随年龄的变化曲线用三次回归分析模型拟合优度最佳 ,其峰值在 3 5～ 3 9岁 ,40岁后OP检出率随增龄而增加。Objective To cross-sectionally investigate the age-related variation of speed of ultrasound (SOU) at tibia in women from Changsha area in China, and to establish the reference database of diagnosing osteoporosis (OP) by quantitative ultrasound (QUS). Methods SOU at the middle of right tibia was measured by QUS Sound Scan 2000 device in 1596 women aged 12-96 years. Results Data were analyzed in age groups divided by intervals of every 5 years. Peak SOU at tibia was (3991.4±68.0)m/s and appeared in the 35-39 years group. The SOU value was increased with a geometric mean (G) of 1.41% every 5 years before 35 years, and declined with G of 1.07% every 5 years after 40 years. This curve of SOU change with age was best-fit goodness by regression analysis of cubic equation 〔SOU=3383+39.9×(age)-0.78×(age) 2+0.0039×(age) 3 (R 2=0.505,P<0.001)〕. Theprevalencesofosteoporosisinthese women were 4.7% in 40-49 years, 27.0% in 50-59 years, 62.2% in 60-69 years, 79.6% in 70-79 years and 89.7% in over 80 years groups. Conclusion The curve of tibia SOU with age is best-fit goodness by regression analysis of cubic equation in these women. The peak value of SOU at tibia appears in the 35-39 years group. The detection rate for osteoporosis is found increasing with age after 40 years old.
