摘　　要：目的 :探讨WOLTER锁骨钢板治疗锁骨远侧端骨折和肩锁关节脱位的临床应用效果。方法 :2 0 0 1年 3月～ 2 0 0 3年 5月间 ,应用WOLTER锁骨钢板治疗锁骨远侧端骨折 12例 ,肩锁关节脱位 8例 ,锁骨远侧端骨折并肩锁关节脱位 7例。男 17例 ,女 10例 ,年龄 18～ 5 8岁 ,平均 3 7 1岁。术前JOA评分 2 8～ 5 4分 ,平均 40 1分。结果 :术后随访 4～ 18个月 ,平均 11 7个月。术后JOA评分 75～ 96分 ,平均 85 4。所有病例无术中并发症发生 ,术后 48h允许病人肩关节运动 (前屈、外展 90°)。 8周后除 1例病人外 ,肩关节运动均达到正常范围。JOA评分较术前提高 ,差异有显著意义 (P <0 0 1)。结论 :WOLTER锁骨钢板为治疗锁骨远侧骨折和肩锁关节脱位提供了一种新方法。Objective:To discuss the clinical result of WOLTER clavicular plates for treatment of distal clavicular fracture and acromioclavicular luxation.Methods:From March 2001 to May 2003,27 patients underwent WOLTER clavicular plates for 12 patients with distal clavicular fracture,8 patients with acromioclavicular luxation,7 patients with distal clavicular fracture and acromioclavicular luxation.There were 17 males and 10 females.The average age of the patients at surgery were 37.1 years (ranged,18 to 58 years).The average preoperative JOA score were 40.1 points (ranged,28 to 54 points).Results:The average period of follow-up were 11.7 months (ranged,4 to 18 months).The average postoperative JOA score were 85.4 points (ranged,75 to 96 points).There was no complication occurred during operative or postoperative periods.Patients could move their should joint postoperative 48 hours.All the patients can recove function of should joint like preoperative except 1 case.The JOA score was increased significantly compared with that of preoperative ( P<0.01 ).Conclusion:WOLTER clavicular plate is a new operative for the treatment of distal clavicular fracture and acromioclavicular luxation.
