摘  要:目的 对同种异体尿道细胞外基质 (ECM)重建尿道的功能进行评估 ,寻找理想的尿道替代材料。方法 将 5只兔阴茎部尿道摘除后进行脱细胞处理 ,制成尿道细胞外基质 ,用电镜检测脱细胞效果 ,观察微观结构。另 15只兔分 3组 ,分别是尿道ECM移植组 (实验组 )、异体尿道移植组 (对照组 1)和自体尿道再生组 (对照组2 ) ,术后对比研究重建尿道的形态与功能。结果 实验组术后尿流通畅。 6周取材大体观察见ECM区尿道壁血运良好 ,12周尿道造影见尿道显影正常 ,无明显狭窄 ,无结石形成。对照组 2的尿道缺损区被周围组织和纤维包裹而明显增粗成囊腔。Objective To experiment the possibility of extracellular matrix for the replacement of urethra.Methods Extracellular matrix was obtained from the urethras of 5 rabbits by decellularization process.15 experimental rabbits were used and 1cm segment of urethra was resected in each.In the experimental group of 5 rabbits,the resected urethra was replaced by the ECM.In the first control group of 5 rabbits the resected urethra was replaced by allograft transplantation of untreated urethra.In the second control group the resected urethra was left untreated for autoregeneration.The neourethra segment was studied.Results In the experimental group,the wall of urethra in the ECM area was red 6 weeks after replacement.No stenosis was observed on urethrography 12 weeks after replacement of the resected urethra.In the second control group,the studied area was embeded with fibrosis tissue.Conclusion Allourethral ECM might be an ideal replacement material for urethral reconstructure.
