逆行浅静脉-皮神经营养血管岛状皮瓣的临床应用

摘  要:目的 :探讨 (前臂、小腿 )逆行浅静脉 -皮神经营养血管岛状皮瓣的血供 ,手术方法及临床应用效果。方法 :用逆行头静脉 -前臂外侧皮神经营养血管岛状皮瓣修复腕部及手背软组织缺损 6例 ,逆行小隐静脉 -腓肠神经营养血管岛状皮瓣修复踝及足跟周围软组织缺损 8例。结果 :14例中 12例皮瓣全部成活 ,2例远端小部分坏死 ,随访 6~ 18个月 ,皮瓣质地优良 ,外观及功能满意。结论 :前臂及小腿逆行浅静脉 -皮神经营养血管岛状皮瓣血供可靠 ,切取容易 ,不牺牲主干动脉 ,疗效满意。逆行头静脉 -前臂外侧皮神经营养血管岛状皮瓣可满意地修复腕及手背部软组织缺损 ;逆行小隐静脉 -腓肠神经营养血管岛状皮瓣是修复踝及足跟周围软组织缺损的理想供体。Objective:To report the results of clinical application of retrograde transposition distally based veno-neurocutaneous vascular island flap in the extremities and to investigate the blood supply of veno-neurocutaneus vascular flap.Methods:Six cases with soft tissue defects around the wrist and dorsum of the hand were treated with cephalic veno-lat.antebrachial cutaneous neuro-vascular island flaps,and another 8 cases with soft tissue defects around the lateral side of the ankle and the heel were treated with distally based small saphenous veno-sural neuro -vascular island flaps.Result:The flaps totally survivied in 12 cases,and the others had small distal neurosis.The color and texture of the flaps were exrtremely good,and their appearance and function were satisfactory after 6~18months followup.Conclusion:The retrograde transposition of distally based veno-neuro-cutaneous vascular island flaps had the reliable blood supply and the avoidance of sacrificing the major arteries of the extremity and was easy to harvast the flap.The distally based cephalic veno-lat.antebrachial cutaneous nevo-vascular island flaps was satisfactory in repairing the soft tissue deffects around the wrist and the dorsum of the hand ,and the distally based small saphenous veno-sural neuro-vascular island flap was an ideal flap for repairing the soft tissue defects around the ankle and the heel.
