大黄导泻在急性有机磷农药中毒救治中的临床价值

摘　　要：目的　观察大黄导泻在抢救急性口服有机磷农药中毒 (AOPP)患者中的作用 ,评价大黄导泻在预防 AOPP合并反跳、中间综合征 (IMS)、多脏器功能障碍综合征 (MODS)及缩短住院时间、提高治愈率中的价值。方法　将 6 2例急性口服有机磷农药重度中毒患者随机分为大黄导泻组 (A组 ,31例 )及对照组 (B组 ,31例 ) ,其年龄、性别、病情等无显著差异 (P>0 .0 5 )。两组入院后均予常规治疗 ,在此基础上 ,A组取大黄粉 2 0 g溶于 2 0 0 ml开水中待温 ,于每次洗胃后由胃管注入 ,每 8小时 1次 ,连续 3天 ;对两组全血胆碱酯酶 (CHE)、心肌酶进行动态监测并观察住院时间及有无 IMS、反跳、MODS发生。结果　入院第 2天 A组及 B组血 CHE、心肌酶比较有极显著性差异(P<0 .0 1) ,A组住院时间和反跳、IMS、MODS发生率明显低于 B组 (P均 <0 .0 5 )。结论　大黄导泻有利于提高AOPP患者全血 CHE、改善心肌酶 ,减少反跳、IMS、MODS的发生 ,缩短住院时间 ,明显提高治愈率。Objective To observe the function of rushing down with rhubarb for the acute organophosphorus poisoning(AOPP) patients through the mouth and to evaluate the value of rushing down with rhubarb on preventing the repeating condition,intermediate syndrome(IMS),multiple organ dysfunction syndrome(MODS),decreasing the time in hospital and improving the cure rate.Methods 62 acute organophosphorus serious poisoning patients through the mouth were randomly divided into experimenting group(group A,31 cases) and control group(group B,31 cases).All patients were given the routine therapy,additional,the patients in group A were given rhubarb once every 8 hours.Rhubarb powder 20 grams was dissolved among 200 milliliters of boiling water.When it became warm,it was poured into stomach from the stomach tube after washing the stomach.It takeed 3 days in a succession.Later,the blood cholinesterase(CHE) and myocardium enzyme of the two groups were measured dynamically.At the same time,the days in hospital,the repeating condition,IMS and MODS were observed.Results There were much significant difference in the blood CHE and myocardium enzyme between group A and group B from the second day being in hospital(P<0.01).The days in hospital and occurrence rates of the repeating condition,IMS and MODS in group A were significantly lower than that in group B(P<0.01,<0.05).Conclusion For the serious AOPP patients,the thepary of rushing down with rhubarb can improve the blood CHE,ameliorate the myocardium enzyme,decrease the time in hospital and the occurrence rate of the repeating condition,IMS,MODS and improve the cure rate.
