单纯性肥胖患者血清IL-6水平的变化及西布曲明减重治疗对其的影响

摘　　要：目的 :观察单纯性肥胖患者血清 IL- 6水平及西布曲明减重治疗对其的影响。 方法 :4 5例单纯性肥胖患者分别接受西布曲明 (2 5例 )和安慰剂 (2 0例 )治疗 2 4周 ,治疗前后测量患者的体质量、腰围、空腹血糖、胰岛素及 IL- 6的浓度 ,计算体质量指数 (BMI)和胰岛素抵抗指数 (HOMA- IR) ,并与正常对照组 (1 6例 )进行比较。 结果 :单纯性肥胖患者的体质量、BMI、腰围均明显高于正常对照组 (P<0 .0 0 5 ) ,经过 2 4周治疗后 ,单纯性肥胖患者的体质量、BMI和腰围均有明显下降 (P<0 .0 0 5 ,P<0 .0 5 ) ,但西布曲明治疗组下降的程度明显高于安慰剂组 (P<0 .0 0 5 ) ;与正常对照组比 ,单纯性肥胖患者空腹血清 IL- 6、胰岛素水平及 HOMA- IR明显升高 (P<0 .0 0 5 )。西布曲明治疗后 ,血清 IL- 6、胰岛素水平及 HOMA- IR较治疗前明显下降 (P<0 .0 0 5 ) ,而安慰剂组血清胰岛素水平及 HOMA- IR较治疗前下降 (P<0 .0 5 ) ,IL- 6有下降趋势 ,但 P>0 .0 5。单纯性肥胖患者血清 IL- 6水平与 BMI、腰围、空腹胰岛素及 HOMA- IR密切相关 (P<0 .0 5 ,P<0 .0 0 5 ,P<0 .0 1 )。西布曲明组的血清 IL- 6的下降与 BMI、HOMA- IR的下降呈明显正相关 (P<0 .0 5 ) ,与腰围下降相关性更大 (P<0 .0 1 )。 结论Objective:To study the change of serum interleukin 6 levels in obese patients and effect of sibutramine on its levels.Methods:Totally 45 obese patients were assigned into sibutramine treated group( n =25) and placebo treated group( n =20), and 16 normal weight subjects were taken as control group.Before and after 24 week treatment,body mass,waist circumferences,serum levels of interleukin 6,glucose and insulin were determined,and the body mass index(BMI) and HOMA IR were calculated.Results:(1) Body mass,BMI and waist circumferences decreased after 24 week treatment in sibutramine and placebo group( P <0.005, P <0.05),and those of the sibutramine treated patients were more obviously in comparison with those of placebo treated patients.(2) Compared with normal subjects,fasting serum levels of interleukin 6,insulin and HOMA IR increased significantly in obese patients( P <0.005).After 24 week treatment,obese patients showed significant decreases of interleukin 6,insulin and HOMA IR( P <0.05),especially in sibutramine treated groups.(3)Fasting serum interleukin 6 levels were positively correlated with BMI,waist circumferences,insulin and HOMA IR in obese patients.Interleukin 6 reduction was positively correlated to the reduction of BMI( P <0.05),especially waist circumferences( P <0.01) in sibutramine treated groups.Conclusion:Serum interleukin 6 levels is increased in obese patients.Sibutramine induced mass and intra abdominal fat loss in obese patients reduces serum interleukin 6 levels.
