摘 要:目的 :探讨以 3次尿红细胞位相显微镜检查作为血尿的定位诊断方法。以尿红细胞 >80 0 0个 /ml,其中畸形红细胞 >70 % ;G1红细胞≥ 5 % ,作为肾小球性血尿的诊断标准 ,评估其准确性及临床应用价值。方法 :用双盲法研究 12 8例血尿患者 ,其中 6 6例 (经肾活检证实 )为肾小球疾病 ,6 2例为非肾小球性疾病。通过评估患者一次尿红细胞位相显微镜及三次尿中任意一次尿红细胞位相显微镜检查达到肾小球性血尿诊断标准与肾小球疾病的符合率 ,计算出其灵敏度和特异度。结果 :此诊断标准的灵敏度为 90 .9% ,特异度为 96 .8%。结论 :与以往的报道比较 ,本诊断方法对肾小球性血尿定位诊断的准确性更好 ,而且简便易于掌握 ,适于临床应用。Objective:To explore the accuracy and clinical value of examining morphology of red cells in urine by phase-contrast microscopy in localization of hematuria. The diagnostic standard of glomerular haematuria was red cells> 8 000/ml, with deformity red cells>70% and G 1≥5%.Methods:128 patients with haematuria were examined by double-blind method, including 66 cases with glomerular diseases(confirmed by kidney biopsy) and 62 cases of non-glomerular disease. The accordance rate of red cells in urine examined by phase contrast microscopy with the diagnostic standard of haematuria one and among 3 times and the sensitivity and specificity was analyzed.Results:The sensitivity of this diagnostic standard was 90.9% and the specificity was 96.8%.Conclusion:Compared with the previous reports, the accuracy of this diagnosis method is more satisfactory in glomerular haematuria. It is easy to master and suitable for clinical diagnosis.
