过敏性鼻炎口服液治疗常年性变态反应性鼻炎的临床研究

摘　　要：【目的】探讨具有健脾补肺,益气固表,祛邪通窍作用的过敏性鼻炎口服液治疗常年性变态反应性鼻炎(PAR)的临床疗效及可能的作用机制。【方法】将符合PAR诊断的129例患者随机分配到观察组(43例)、对照组(43例)和安慰组(43例),观察组予过敏性鼻炎口服液(由桂枝汤合玉屏风散加味而成),对照组予鼻炎康片,安慰组予生理盐水。3组均以4周为1疗程,停药1周后再服1个疗程。两个疗程后判断疗效,并观察各组血清白细胞介素4(IL-4)和血清特异性IgE抗体(SIgE)的变化情况。【结果】3组的总有效率分别为93.02％、65.12％、6.98％,观察组和对照组的疗效均优于安慰组(均P<0.05),其中尤以观察组的疗效为佳(与对照组比较,P<0.01);治疗后观察组血清IL-4、SIgE含量水平有明显改善(与对照组和安慰组比较,均P<0.01)。【结论】过敏性鼻炎口服液对常年性变态反应性鼻炎(PAR)的治疗作用优于鼻炎康片;研究结果还表明,过敏性鼻炎口服液能有效治疗PAR,可能与其降低血清IL-4、SIgE含量水平有关。[Objective] To observe the effect of Allergic Rhinitis Oral Liquid (AROL) on perennial allergic rhinitis (PAR) and to explore its therapeutic mechanism. AROL, composed of modified Guizhi Tang and Yupingfeng San, has the actions of strengthening spleen and lung, replenishing Qi and consolidating superficies and dispelling pathogens and dredging orifices. [Methods] One hundred and twenty-nine cases of confirmed PAR were randomized into Group A (n=43), Group B (n=43) and Group C (n=43). Group A was treated with AROL, Group B with Biyankang Tablets and Group C with normal saline. Three groups were treated for 4 weeks as one treatment course, and another treatment course was carried out one week after interval. Therapeutic effect were observed after treatment and serum levels of interleukin-4 (IL-4) and specific IgE antibody were detected. [Results] The total effective rate was 93.02% in Group A (P<0.01 compared with Group B and P<0.05 compared with Group C), 65.12% in Group B ( P<0.05 compared with Group C) and 6.98% in Group C. Serum levels of IL-4 and specific IgE antibody were much improved in Group A (P<0.01 compared with Group B and Group C). [Conclusion] The therapeutic effect of AROL is better than that of Biyankang Tablets in treating PAR and its therapeutic mechanism may be related to the decrease of serum IL-4 and specific IgE antibody.
