不同时辰、不同穴位、不同频率电针治疗大白鼠实验性胃溃疡的疗效观察

摘　　要：针灸效应是多种因素共同作用的结果,从多水平上研究各种因素对针灸效应的影响,有利于揭示各种因素与针效之间的内在联系。为此,本文应用拉丁方设计法研究了三个时表(亥、酉、巳时)三个穴位(足三里、梁门、胃俞)和三种频率(2HZ,20HZ,80HZ)的电针对okabe法造模的大白鼠实验性胃溃疡修复和愈合的影响。观察结果显示:①各电针治疗组的平均溃疡面积、容积、炎细胞浸润量、坏死组织存在例数、胶原纤维增生和粘连程度均显著小于对照组(P<0.01);2.亥时电针治疗能明显促进溃汤修复和愈合,其疗效优于酉、巳二时辰(P<0.01),而后两者间无显著性差异;3.足三里、梁门、胃俞三穴均为电针治疗大白鼠实验性胃溃汤的有效穴,三者间无统计学差异(P>0.05),但以梁门穴为优;4.三种频率对电针效应的影响无显著性差异(P>0.05),但以20HZ电针的效果较佳;5.三因素三水平的不同组合对针效的影响各不相同,其中以亥时梁门穴20HZ电针的治疗效果为佳。研究表明,将多种因素在不同水平上优化组合是提高针灸疗效的有效途径,并进一步证实了“时间针法”的客观性,从而为针灸原理的研究和提高临床疗效提供了实验依据。Acupuncture effects result in a combined action of various factors. It is beneficial to revealing the immanent relationship between effect and factor to investigate simultaneously the in- fluence of different factors with different levels on the therapeu- tic effects of acupuncture. Hence effects of electroacupuncture electroacupuncture at three Shi Chen (Hai Shi, Si Shi and You Shi), three points (Zusanli, Liang men and Weishu points) and kinds of frequency (2, 20 and 80Hz) on the healing of experi- mental gastric ulcer in rats, which was caused by Okabe method, were studied by Latin Square Design method. It was found that: (1) the mean values of the area and vol- ume of the ulcer, the number of inflammatory cell infiltration, case of existance of necrosis tissues and the severity of adhesion and proliferation of collagenous fibers in electroacupuncture group were statistically significant less than that in the control group (P<0. 01); (2) electroacupuncture treatment at Hal Shi could markedly facilitate the repairing and healing of the ulcer, and the therapeutic effects in Hai Shi group were significant bet- ter than that in both Si Shi and You Shi groups (P<0. 01), but the difference in therapeutic effects between Si Shi and You Shi groups was not significant; (3) Zusanli, Liangmen and Weishu points all were effective points to treat experimental gastric ul- cer in rats through electroacupuncture, and although the differ- ence in therapeutic effects among three groups was not signifi- cant (P>0. 05), Liangmen point was the best one; (4) the acupuncture effects among three frequency groups were not markedly different (P>0. 05), however, the effect in 20Hzgroup seemed to be the best; (5) effects of elec- troacupuncture of different combinations of three factors with three different levels in terms of Latin Square Design were dif- ferent, but the effect of the combination of Hal Shi, Liangmen point and 20Hz frequency was found to be the best.
