摘  要:目的 探讨改良生理学和手术严重度评分检点并发症和死亡率(POSSUM)评分系统对高龄烧伤患者术后并发症评估的护理应用意义。方法回顾性分析70例≥60岁患者的术后并发症情况,用改良POSSUM评分系统进行评分,并按Copland并发症发生率计算公式,比较实际并发症人数与预测发病数。结果 有并发症19例,平均年龄(69.1±3.4)岁,平均住院时间(45.6±6.8)d,生理学评分(23.8±5.7),手术侵袭度评分(12.1±4.3)。无并发症组51例,平均年龄(68.3±2.5)岁,平均住院时间(27.6±9.7)d,生理学评分(19.4±4.5),手术侵袭度评分(10.1±3.2),差别有显著意义(P<0.05)。按Copland公式预测并发症人数22例,实际人数19例,差别无显著意义(P>0.05).Copland公式计算患者的并发症率与实际并发症相符。结论 改良POSSUM评分系统对预测高龄烧伤患者术后并发症的发生有重要的临床护理意义。Objective To evaluate clinical important of modified POSSUM surgical scoring system in for aged patients with burns.Methods 70 cases patients were retrospectively analyzed with the modified POSSUM surgical scoring system.A comparison between the observed and the predicted complication were made.Results In the complication group and no complication group, the mean age of the patients were 67.1±3.4 and 67.3±2.5, the mean length of stay were 44.6±6.8d and 6.6±9.7d.respectively.In the complication group, both the physiological scores and operative severity scores on POSSUM scoring system were significantly higher than that in no complication group 22.8±5.7 vs 18.4±4.5 in physiological score and 11.1±4.3 vs 10.9±3.2 in operative severity score, (P<0.05).There was perfect agreement between the observed and the predicted morbidity as by calculated published predictor equation for morbidity.Conclusions Modified POSSUM scoring system may be appropriately used to predict tne occurrence of complication for aged patients suffering burns.It play a important role in guiding clinical care and decreasing complication.
