腹腔镜子宫切除术126例分析

摘　　要：目的 分析126例腹腔镜子宫切除术(LH),探讨LH的适应证、手术要点和并发症的预防。方法 126例因各种妇科良性疾病需行剖腹全子宫切除术(AH)的患者,行腹腔镜子宫切除术。术前盆腔检查和超声发现21例患者子宫正常大小,余105例均超过正常,其中≤12孕周者24例,12～16孕周者65例,16～18孕周者16例。有下腹部手术史者54例。结果 126例LH患者均顺利进行,平均手术时间104(62～180)min,术中出血量120(30～250)mL,术后不需应用镇痛药,术后平均住院日3.5d。结论 LH可替代部分AH。手术者必须具备阴道手术和腹腔镜手术技术,选择适应的患者,从而减少剖腹手术。.Objective To evaluate the indications, technical keypoints and prevention of complications of laparo-scopic hysterectomy ( LH) . Methods One hundred twenty-six women who suffered from various kinds of benign gynecologic diseases underwent LH. Pre-operative pelvic examination revealed that the uterus were normal in 21 cases , enlarged less than 12 weeks gestational size in 24 cases, between 12 and 16 weeks gestational size in 65 cases, and-between 16 and 18 weeks gestational size in 16 cases. Among these patients, 54 cases had a history of lower abdominal laparotomy. Results The mean operation time of LH was 104 min ( rang 62-180 min). The mean amout of blood loss was 120 mL( rang 30-250 mL) and the average postoperative hospital stay was 3.5 days. Conclusion Laposcopic hysterectomy (LH) is an acceptable substitute for abdominal hysterectomy. To safely and efficiently complete LH , the surgeon must be experienced in both vaginal surgery and advanced operative laproscopy.
