摘  要:目的 了解老年女性糖尿病骨质疏松症维生素D受体基因的分布及骨钙素的变化情况。方法 用聚合酶链反应限制性片段长度多态性 (polymerasechainreaction restrictionfragmentlengthpolymporphism ,PCR RFLP)技术检测受试者维生素D受体基因 ;采用单光子骨密度仪测定受试者非优势前臂中下 1 / 3交界处桡骨骨密度 (BMD)。结果 对照组维生素D受体基因型bb、Bb、BB分别为 86 4 %、1 3 6 %、0 % ;糖尿病非骨质疏松组分别为 85 %、1 5 %、0 %。糖尿病骨质疏松组分别为 85 7% ,1 4 3% ,0 % ,3组比较无统计学意义P >0 0 5 ,骨钙素水平为糖尿病骨质疏松组 <糖尿病非骨质疏松组 <对照组 ,3组比较均有统计学意义P <0 0 5 ,糖尿病骨质疏松组空腹胰岛素亦较其余两组低。结论 老年女性糖尿病骨质疏松与BB基因型无明显相关 ,但与胰岛素水平及骨钙素水平有关 ;胰岛素可能是调节骨形成的重要因子 。Objective To determine the frequency distribution of vitamin D receptor(VDR)genotype and changes of serum BGP in diabetic patients with osteoporosis in old women.Methods Polymerase chain reaction(PCR) and restriction analysis were used to determine VDR genotypes in all objects,their under one third of radioulnar in no dominant arm BMD was measured by single photon absorptimetry(SPA).Results The frequency distribution of the VDR genotype in control group:bb,Bb,and BB genotype accounted for 86.4%,13.6% and 0% respectively;in no osteoporosis with diabetic patients,they were 85%,15%,and 0% respectively;in osteoporosis with diabetic patients they were 85.7%,14.3%,and 0%( P >0.05).Serum BGP levels:osteoporosis with diabetic patients<no osteoporosis with diabetic patients<control group ( P <0.05).Conclusion There was no association between BB genotype and osteoporosis with diabetic patients in old women.There were correlarions between insu/in BSP and ostevporosis with diabetic patients;maebe insu/in was an important factor.But it is still a problem whether BGP can give a forecast of osteoporosis,or be a marker of state of diabetes.
