氯沙坦对高血压病患者血管内皮功能的影响

摘　　要：目的　观察氯沙坦对高血压病患者血管内皮功能的影响 ,并与血管紧张素Ⅱ和内皮素的变化对比。方法　 84例高血压病患者随机分成两组 ,根据血压高低予药物治疗。氯沙坦组 (A组n =42 )予氯沙坦 5 0～ 10 0mgQd ;硝苯地平缓释片组 (B组n =42 )予硝苯地平缓释片 10～ 2 0mgBid ,两组治疗疗程均为 8周。治疗前及治疗后 8周予超声检测肱动脉内皮功能和测定血浆血管紧张素Ⅱ (AngⅡ )及内皮素 (ET)的浓度。结果　肱动脉内径基础值及含服硝酸甘油前后肱动脉内径变化无显著差异 (P >0 .0 5 ) ,反应性血管充血引起肱动脉内径变化治疗前后两组均有显著差异 (P <0 .0 5 ) ,且A组优于B组 (P <0 .0 5 )。A组治疗后血浆AngⅡ水平升高 (P <0 .0 1) ,ET水平下降 (P <0 .0 1)且优于B组 (P <0 .0 5 ) ,而B组治疗后血浆AngⅡ水平无显著差异 (P >0 .0 5 ) ,而ET水平下降 (P <0 .0 5 )。结论　氯沙坦在有效降压的同时 ,能有效改善血管内皮功能 ,且优于硝苯地平缓释片。Objective To study the effect of Losartan on the endothelium-dependent relaxion-compare with level of angiotensionⅡ and endothelin-1 in patients with hypertension.Methods Eighty-four patients with hypertension were divided randomly into 2 groups: according to the level of blood pressure for 8 weeks,losartan group (group A n=42) was treated with Losartan(50-100mg 1/d);Nifedipine SR group(group B)was treated with Nifedipine(10-20mg 2/d). with high resolution untrosound,we measure endothelium-dependent relaxing function,contrasted with levels of angiotensionⅡ and endothelin-1 after 8 weeks.Results There were no significantly different in basefine and response to nitroglycerin(P>0.05).There were significantly different in responsive congestion to brachial artery before and afer the treatments of Losartan or Nifedipine SR.(P<0.05),and group A was better than group B(P<0.05). It was obvious inceasing in levels of angiotension Ⅱ and decreasing in levels of endothelin-1 afer treatment of Losartan.It was abvious no different in the levels of angiotension Ⅱ and decreasing in levels of endothlin-1 after treatment of Nifedipine SR. what's more ,group A have more effective on than group B.Conclusions Losarfan and Nifedinpine SR can decrease blood pressure stabilizely.and losartan have a benefit on endothelium-dependent vasodi lation function and have more effective than Nifedipine SR.
